I, _ _ Denis Santos _ the condominium unit owner,
understand that It Is my responsibility to obtain whatever permits are required by the St. Lucie County
Building Department for my proposed alterations, and to comply with all local and State building codes
and ail requirements and municipal regulations relative to contractor licensing.
I understand that my contractors must check in with the Association's Representative upon arrival each
day, and that they are subject to all Association rules and subject to all associated fees for violations.
I understand that I am responsible for the cost of repairs to any and all damage done to the
condominium building or grounds by my contractors.
I understand that it is my responsibility to make sure that my contractors are aware of and in agreement
with the following requirements for work to be done.
• Nothing but sanitary waste may be disposed of in the sanitary lines.
• Nothing may be removed or discarded out the windows or off of balconies.
• All dust and debris must be contained within the unit.
• All garbage and debris must be removed by the contractor daily, and not by way of dumping
construction debris Into the condominium's garbage dumpsters,
• Management shall be permitted to check on and take photographs of any work site before and
in any stage of construction being done on the Atlantis Building A Condominium.
• Excessive noise, misconduct of workers, improper parking of construction vehicles, or violation
of any condominium rules may be grounds for Proactive Property Management to revoke the
construction approval and/or take any action that is deemed appropriate by the board of
Directors.
• Construction work hours are permitted Monday- Friday from 8am — 5pm and Saturdays gam -
5pm, Sundays are not permitted,
• Prior notice must be given to the Association's maintenance man, with time to alert those above
and below of such work.
• dress code shall include shirts and closed toed shoes.
• A fine of $100 will be assessed to anyone that parks on the right of way, in swales, or other
common areas not designated for parking.
• Children and animals are not permitted on work sites.
• PPM/ARC has 30 days from the date of submittal to make its recommendations, but undo delays
are to be avoided.
• A written letter of approval will be given to the owner as formal verification of approval or
disapproval.
• PPM reserves the right to require the posting of a construction bond between $500 - $1,500 to
be held in escrow as protection for the Condominium Association from damage to the condo
building or common areas during the construction process.
ARCHITECTURAL REVIEW PROCESS
Applicants must submit a copy of this application to Proactive Property Management, on behalf of the
Board of Directors, for the Architectural Review Committee (ARC).
The applicant (or designated representative) may be required to attend a meeting to review and discuss
submittal.
Please submit the following in order that your request may be reviewed.
1. A detailed written description of the scope of proposed alterations (on the bottom of this page).
2. A copy of the "Proposed Alteration Application" form, signed by the owner and any contractors
hired to do the proposed work.
3. A copy of the contractor's license, insurance and workers compensation with the Atlantis A
Condominium Association, INC C/O Proactive Property Management North, INC in the certificate
holder's box.
4. $50 application fee made out and mailed to Proactive Property Management North 11198 Polo
Club RD Wellington FL 33414.
Checklist must be completed prior to acceptance of application for ARC Committee Review.

Detailed description of scope of proposed Alterations: Add connections for instalments of washer
and dryer in the second bedroom closet. Plumbing will be obtained from second bathroom .
PPM reserves the right to ask for copies of building permits, verification of contractors' licenses,
and certificates of insurance, listing "Atlantis Building A Condominium Association, Inc. and
Proactive Property Management North, INC" as additional insured.
