This survey was conducted in accordance with Chapter 61-145 of the United States Environmental Protection Agency’s (USEPA) Title 40, Part 61, Subpart M of the Code of Federal Regulations (CFR). This report includes the results of our findings from visual reconnaissance, field sample collection and laboratory analysis. This Asbestos Containing Materials Building Survey in general accordance with the scope and limitations of NESHAP and UES’s proposal dated December 10, 2021 and approved by Retrofits and Renovations on December 14, 2021. B APPENDIX C Certifications ……………………………………………………………………………………………………………………………C Asbestos Containing Materials Building Survey Report UES Project No. 3440.2100133.0000 Building located at 4625 South Indian River Drive Fort Pierce, St. Lucie County, Florida January 5, 2022 Page 1 1.0 INTRODUCTION 1.1 General The purpose of this Asbestos Containing Materials Building Survey was to identify and assess suspect asbestos containing materials (ACM) within one building structure located at 4625 South Indian River Drive in Fort Pierce, St. Lucie County, Florida. UES understands that the building may be slated for renovation/demolition. 1.2 Purpose and Scope of Work The purpose of this study was to perform an evaluation of the above-referenced facility for the presence of ACM, specifically those building surfaces/components which may be present or impacted during potential renovation/demolition activities. The activities and procedures used to accomplish this task were as follows: 1) Review available information concerning the subject building including the dates of initial construction, significant renovations, types of construction, and information regarding the on-site structures uses; 2) Visual building inspection of accessible areas by an inspector to identify, locate, and assess suspect ACM; 3) Once identified, collect representative samples of suspected ACM for laboratory analysis using polarized light microscopy; 4) Prepare and submit a report. Complete destructive observation and sampling procedures were not generally used in UES’ evaluation of the subject building. Inaccessible areas within the building, such as crawl spaces, inside other sealed areas, behind architectural details are beyond the scope of this study. The scope of UES’ assessment did not include an evaluation of the structure’s fixtures, equipment, or stored materials. 2.0 SURVEY METHOD On December 23, 2021, Robert Crowell, a certified Asbestos Hazard Emergency Response Act (AHERA) Asbestos in Buildings Inspector, conducted an asbestos sampling survey of suspect asbestos-containing building materials associated with the pending renovation/demolition of the building. A Site Location Map is included as Figure 1. Building materials associated with the subject building proposed for renovation/demolition were inspected and sampled as part of this survey. During the NESHAP asbestos sampling survey conducted at the subject building, the inspector collected a total of ten (10) bulk samples from six (6) identified homogeneous areas of suspect asbestos-containing Asbestos Containing Materials Building Survey Report UES Project No. 3440.2100133.0000 Building located at 4625 South Indian River Drive Fort Pierce, St. Lucie County, Florida January 5, 2022 Page 2 materials. The samples were delivered to Eurofins EMLab P&K, Fort Lauderdale, Florida, a certified asbestos laboratory for analysis of asbestos by polarized light microscopy (PLM). Field Logs indicating the homogeneous area, sample number, description, location and NESHAP Category of each sample collected are provided in Appendix A. 3.0 SURVEY RESULTS The laboratory report and chain-of-custody documentation are included in Appendix B. Asbestos mineral fibers were not detected above 1 percent in any of the building material samples collected and tested. UES was not informed of additional building materials that existed at the subject property prior to this survey. If additional materials that have not been tested are discovered during renovation activities or if any materials are found that were not accessible at the time of the survey, they should be considered Asbestos Containing Materials Building Survey Report UES Project No. 3440.2100133.0000 Building located at 4625 South Indian River Drive Fort Pierce, St. Lucie County, Florida January 5, 2022 Page 3 Presumed Asbestos Containing Materials (PACM) in accordance with Chapter 1910.1001 of the Occupational Safety and Health Administration’s (OSHA) Title 29, Part 1910, Subpart Z of the CFR, until laboratory testing proves otherwise. In addition, the contractor should notify the subject property owner and UES of any such materials for further evaluation. A copy of the survey should be kept onsite during renovation/demolition activities. This report may not be used by the client to claim product endorsement by National Voluntary Laboratory Accreditation Program (NVLAP) or any agency of the U.S. Government. Method used: EPA Method for the Determination of Asbestos in Bulk Building Materials (EPA/600/R-93/116 July 1993). Copies of relevant UES Asbestos Inspector Certifications are included as Appendix C. 4.0 CONDITIONS AND LIMITATIONS OF THIS SURVEY A representative of UES performed the asbestos containing materials building survey on December 23, 2021. This survey is applicable for the time that the assessment was conducted. Inaccessible areas within the building located at 4625 South Indian River Drive, such as inside partitions, under sealed, bolted metal roof panels, inside crawl spaces, inside other sealed areas and behind architectural details are beyond the scope of this study.
