As requested, my associate Jeffrey Miller was on the job site for the installation of 8-
14"diameter, 30 feet long cast -in -place augur piles for the above referenced project.
These piles were reinforced with (5)-#5 bars x 18 feet with #4 ties at 12" o.c.. These
piles were installed on October 9, 2013. '
Each pile was documented for steel installation, # of pump strokes, time, truck # and any
abnormalities during construction. Each pile was checked by "counting the strokes
method" as documented in several literature including "Inspector Guide to the Auger
Cast -in -Place Piles" published by the Auger Cast -In -Place Pile Committee of the" The
Deep Pile Institute"..
The lowest volume pumped in the 8 piles was #7with 27 stroke count which calculates to
34 c.f. which is greater than the exact calculated volume of 32 c.f (see attached
calculations).
Based on this inspection, it is my professional opinion, the piles constructed for the
referenced project were installed as specified in this report and construction drawings to
the best of my knowledge. The pile documentation and location plan are attached.
