Pierce, Florida Re: Visual Site Inspection 5420 Cassia Drive Fort Pierce, Florida KSM 2202418 To Whom It May Concern: Based upon a request from Crystal Chilson, we have been asked to conduct a visual site inspection to determine the approximate footer size. We measured under the footer and found the footer to be generally 10” deep and 12” wide as indicated by random location checks on the north, west, and east sides. The south side did not have access. We have also been asked to conduct a Rebound “Swiss” Hammer test in accordance with ASTM C805 to evaluate the approximate compressive strength of the concrete slab for the above- mentioned address. Representatives from KSM Engineering and Testing (KSM hereinafter) conducted the Rebound Hammer test on March 8th, 2022, at approximately 12:00 PM. Scope of Test: The completion of this test by KSM serve to document and evaluate the condition of the concrete slab at the time of the inspection within a reasonable professional engineering probability and certainty. KSM will select (3) random locations within the slab for Rebound Hamer Testing. All conclusions or recommendations are based upon testing, observations, or information gathered on- site prior to completion of this document. If field conditions change or differ than as stated in this document, it is the responsibility of all parties to contact KSM for additional review. The compressive strength test results listed below are in accordance with ASTM C805 and based on the corrected average rebound numbers gathered on site. Concrete Properties: The following was provided to KSM by the client. Design Mix / Strength: 2,500 psi Headquarters Mailing 11345 U.S. Highway 1 P.O.
