Ocean Drive, Jensen Beach, FL 34957 Contractor: DMF Construction 312 S. Old Dixie Hwy., Jupiter, FL 33458 Permit No. 2003-0510 The completed concrete restoration did not reduce the structural integrity of the remaining existing structural systems and, therefore, a requirement to upgrade them by this permit was not warranted. Existing building's components and elements outside the renovation area of this permit do not have their current wind load resistant capacity or gravity load capacity compromised by the completed concret restoration. Mathers Engineering Corporation performed onsite inspections as the job progressed. Repairs did not require shoring. As the demolition exposed areas of exterior concrete deterioration and steel corrosion of the exterior balcony deck, we initiated and inspected restoration work such as concrete preparation, steel reinforcement restoration, and application of bonding agents, and finished concrete applications. The enclosed Record Drawing (Sheet 1) shows the areas of repair for Stack 1 and is made part of this final close-out document approval under Permit Number 2003-0510. Materials used and work performed to date has been substantially completed in conformance with the specifications outlined in the IRCI Guideline No. 03730, as well as the applicable 2017 Florida Building Codes, 6t' Edition (and updat , other industry guidelines, or standard engineering practices.
