PIERCE,FLORIDA 34949 SLC BUILDING PERMIT NUMBER: 1708-0100 PROJECT NAME: Ocean Harbor North—Bldg A DATE: February 2,2018 CLIENT: Elliott Merrill Community Management LOCATION: Vero Beach,Florida CONTRACTOR: Structure-Con PROJECT MGR: Mr.Khris McConnell INSPECTED BY: Michael Lue WEATHER: Su m), cool I WORK IN PECTED: Units Ojj 11, 611,&711 Concrete slab in this building are reinforced concrete and not a post tensioned slab. I i OBSERVATIONS: All repairs were mostly along the slab edges as indicated in the repair plan. The full depth or surface spalls originally assessed at Unit 511 were not required! The damages were found to hairline cracks in the slab with no corroded reinforcing bars. The cracks in the slab caused failure of the tile bond to the slab which was thought to be spalling concrete. The cracks were epoxied. i I The damages at the slab corners were due to no reinforcing bars along the edge in the existing construction and compounded by the handrail post being in a cored pocket right at the comer of the slab. A#4 reinforcing bar was added along the slab edge at the corner to minimize or alleviate future cracking. RECOMMENDATIONS: All structural repairs have been completed in compliance with the design documents i II ' ,ffflflfll, • NJ NONvp y •, F r < * • r i i r :,'Q.• F G ,o111i11111f4 f i I I I Page 1 of I
