Scates, Professional Engineer (972) 492-9500 April 14, 2016 Overhead Door Corporation 2501 S State Hwy 121 Bus, Suite 200 Lewisville, TX 75067 Re: Evaluation Report -for Series 770 and 790 Sheet Doors FL 12111-R5 To Whom It May Concern: _ At the request of Overhead Door Corporation, 1 have reviewed the drawings and tests listed below. This evaluation was built upon a foundation of actual testing used to validate the engineering calculation methods at known points. Using rational engineering- analysis, the door configurations listed on the drawings were designed to stay within the allowable limits established by the actual door tests. The door components, including the curtain, wind locks, rolled guide, guide angle, and wall fasteners were all verified. I have concluded that the construction shown on these drawings comply with the structural requirements of the Sth Edition (2014) of the Florida Building Code. I certify that I meet the requirements of "independence" as detailed in Florida Statutes. Drawings D-220177 Rev. D D-220185 Rev. C Tests Procedure Series 770 up to 10' wide and 10' tall up to +36.0 / -36.0 psf design Series 790 up to 20' wide and 16' tall up to +41.0 / -41.0 psf design (see chart for specifics) Tests by Architectural Testing (ATI) were conducted at their accredited facility in Southlake, Texas. Tests by Construction Consulting Laboratory, International (CCLI) were conducted at their accredited facility in Carrollton, Texas. Testing was conducted in accordance with ASTM E330-2002. The test process was equivalent to ANSI/DASMA 108-05 (and 108-2012). All tests included holding the door at design pressure for a minimum of 10 seconds (CCLI @ 30 seconds), and achieving a 150% overload ("test" or "proof") j FL 12111-R5 pressure for 10 seconds (CCLI @ 30 seconds). Deflection recovery after completion of the design pressures exceeded 75% on the test doors. 'Pests CCLI Report # CCLI-03-228 ATI Report # 88835.01-801-44 ATI Report # 88833.01-801-44 Installation Instructions The drawings listed above provide installation instructions peculiar to windload reinforcement. Jamb attachment: The door is to be secured to the building using the fasteners specified on the drawings. Additional limitations are itemized in the Notes and elsewhere on the drawings. Architects and other users are reminded that rolling door products can produce substantial "catenary" loads that pull toward the center of the opening during wind load in addition to the normal "perpendicular" forces. The building structure must be designed to withstand the "normal" and "parallel" forces, which are listed in the notes. Additional Limitations The drawings cited above are an explicit part of this evaluation report. The text of this report does not attempt to address all design details, but relies upon the illustrations and text of these drawings and instructions as well. These products were not tested for windborne-debris. These products are not intended for the Florida High Velocity Hurricane Zone (HVHZ). Digitally signed by John E. Scates, P.E.
