In reference to the above project, we have performed a visual inspection of the roof system. We have reviewed and approved the
following product approvals:
1) Flat Roof— Polyglass self -adhered roof system over wood decks NOA#17-1004.09
2) Sloped Roof — Tamko Roof Shingles NOA #15-1215.10 over 60 mil FL 5259-R28 Peel & Stick— Polyglass
The visual inspection included the attic roof surface, ridge, fascia/eve trim, and drip edge. We did not perform any destructive
inspections. The product approvals have been approved and listed in this report. The roof consists of the following:
A. Original Roofing System -Flat Roof
0 1/2:12 Flat Roof
o Manufactured Timber Joists @ 24" OC Max with Structural Plywood Sheathing
o Modified Bitumen over felt over plywood sheathing
B. Original Roofing System - Sloped Roof
0 12:12 Sloped Roof
o Manufactured Timber Trusses @ 24" OC Max with Structural Plywood Sheathing
o Shingles over felt over plywood sheathing
A. New Flat Roof
o Remove existing roof system and underlayment down to bare plywood.
o Remove and replace all damaged structural sheathing with min. 5/8" w/ 10d nails @ 6"OC Max. Zone 1, 4" OC,
Zone 2 & 3
o Install new polyglass underlayment per NOA #F5259-R28
o Install New Flat Roof Per NOA #17-1004.09
B. New Sloped Mansard Roof
o Remove existing roof system and underlayment down to bare plywood.
o Remove and replace all damaged structural sheathing with min. 5/8" w/ 1 Od nails @ 6"OC Max. Zone 1, 4" OC,
Zone 2 & 3
o Install new polyglass underlayment, per NOA #FL5259-R28
o Install New Sloped Mansard Tamko Roof Shingles Per NOA#15-1215.10

mean or imply a guarantee by the engineer nor does an engineer's certification relieve any other party from
meeting requirements imposed by construction, original material strength, or other means, including meeting
commonly accepted industry construction practices. CSM shall not be held responsible for any leaks the roof
may experience. This letter is to show that the installed roof meets or exceeds the loading requirements of the
Product Approval and the FBC 2017; 6ch Edition.
We recommend the roof covering to be approved. All mentioned roof components and attachments appear in
satisfactory condition and have been installed per the Florida Residential Building Code 2017;6th Edition, ASCE
7-10, and the Product Approvals.
