IT IS THE INTENT OF THESE DRAWINGS AND ACCOMPANYING INFORMATION TO DESCRIBE THIS WORK. 2. ALL WORK PERFORMED UNDER THE SCOPE OF WORK CONSIDERED BY THESE DRAWINGS AND ACCOMPANYING DATA SHALL BE IN FULL COMPLIANCE WITH THE REQUIREMENTS OF THE OWNER, FLORIDA DEPARTMENT OF ENVIRONMENTAL REGULATION. AND OTHER AGENCIES WHICH EXERT JURISDICTION. FURTHERMORE, THE CONTRACTOR SHALL BE FULLY RESPONSIBLE FOR ANY AND ALL DAMAGES WHICH MIGHT BE OCCASIONED BY THE CONTRACTOR'S FAILURE TO EXACTLY LOCATE AND PRESERVE ANY AND ALL UNDERGROUND UTILITIES. THE OWNER AND ENGINEER WILL ASSUME NO LIABILITY FOR ANY DAMAGES SUSTAINED OR COSTS INCURRED BECAUSE OF THE CONTRACTOR'S OPERA11ONS IN THE VICINITY OF EXISTING U11LJTIES OR STRUCTURES, NOR FOR TEMPORARY BRACING AND SHORING OF SAME. THE CONTRACTOR SHALL PROVIDE 48 HOURS MINIMUM N0110E TO ALL UTILITY COMPANIES PRIOR TO BEGINNING CONSTRUCTION. THE CONTRACTOR SHALL SCHEDULE AND EXECUTE ALL WORK INVOLVING EXISTING UTILITIES IN ORDER TO MINIMIZE NECESSARY INTERRUPTION OF SERVICES. WHENEVER SUCH INTERRUPTION IS NECESSARY FOR COMPLE11ON OF THE WORK, THE CONTRACTOR SHALL BE RESPONSIBLE FOR NOTIFYING THE ENGINEER AND THE OWNER AT LEAST 48 HOURS IN ADVANCE. ALL WORK TO REPAIR/RESTORE U11UTY SERVICE SHALL BE PERFORMED AS REQUIRED BY THE APPROPRIATE UTILITY. ALL COSTS RELATED TO SERVICE, MAINTENANCE, INTERRUPTION, AND RESTORATION SHALL BE INCLUDED IN THE CONTRACTOR'S BID. 5. IT IS THE CONTRACTOR'S RESPONSIBILITY TO ENSURE HIS COMPLETE FAMILIARITY WITH THE PROJECT SITE AND COMPONENTS TO INCLUDE SUBSURFACE CONDITIONS OF SOIL AND GROUND WATER TABLE. THE CONTRACTOR IS RESPONSIBLE FOR NOTIFYING ALL PERMITTING AGENCIES A MINIMUM OF 48 HOURS PRIOR TO BEGINNING WORK OR PERFORMING TESTS. 7. AFTER CLEARING, THE AREA IS TO BE COMPACTED WITH A HEAVY PLATE VIBRATORY COMPACTOR OR RUBBER TIRED EARTH MOVING EQUIPMENT SUCH AS A LOADER, WITH THE BUCKET FILLED FOR ADDI1IONAL WEIGHT. SUFFICIENT COMPACTIVE EFFORT SHOULD BE APPLIED TO THE CLEARED SURFACE TO OBTAIN A MINIMUM OF 95% OF THE MAXIMUM DRY DENSITY AS DETERMINED BY A.A.S.H.O. T-180 (ASTM D-1557). THE BACKFILL SHOULD BE PLACED AND COMPACTED AS SPECIFIED IN THE FILL PLACEMENT OUTLINED ABOVE. FOOTING EXCAVATION AFTER THE FOOTINGS HAVE BEEN EXCAVATED REPRESENTATIVE DENSITY TESTS SHOULD BE TAKEN. THE DENSITY AT THE FOOTING GRADE SHOULD BE A MINIMUM OF 95% OF THE MAXIMUM DRY DENSITY AS DETERMINED BY A.A.S.H.O. T-180 (ASTM D- 1557). IF THE BOTTOM OF THE FOOTING HAS BEEN LOOSENED DURING EXCAVATION, OR" iF THE BOTTOM OF THE FOOTING IS BELOW 1-FOOT FROM NATURAL GROUND, THE FOOTING AREA SHOULD BE COMPACTED WITH A HEAVY PLATE VIBRATORY COMPACTOR. SUFFICIENT COMPAC11VE EFFORT SHOULD BE APPLIED TO ACHIEVE THE REQUIRED DENSITY. GROUNDWATER SPECIFIC REQUIREMENTS -L INSURANCE - THE CONTRACTOR SHALL BE REQUIRED TO PROVIDE A CERTIFICATE OF INSURANCE NAMING THE OWNER, ENGINEER, ITS ENGINEERS, AND EACH OF THEIR OFFICERS, AGENTS AND EMPLOYEES AS ADDITIONAL INSUREDS UNDER THE CONTRACTOR'S GENERAL LIABILITY POLICY WITH RESPECT TO SERVICES PERFORMED BY THE CONTRACTOR FOR THE OWNER. INSURANCE REQUIREMENTS INCLUDE $1,000,000 GENERAL AND PUBLIC UABILITY COVERAGE. ALSO, THE SUCCESSFUL BIDDER WILL BE REQUIRED TO PROVIDE WORKER'S COMPENSATION INSURANCE FOR ALL PERSONNEL ENGAGED IN THE FULFILLMENT OF CONTRACT OBLIGATION. WORK MAY NOT COMMENCE UNTIL THESE REQUIREMENTS ARE MET. INSURANCE CERTIFICATES DESIGNATING THIS PROJECT AS A SPECIFIED INSURED PROJECT MUST BE SUBMITTED TO THE ENGINEER PRIOR TO CONSTRUCTION. 2. BIDS FOR CONSTRUCTION - THE CONTRACTOR IS REQUIRED TO SUBMIT A SEALED BID FOR THE WORK COVERED BY THESE DRAWINGS AND ACCOMPANYING SPECIFICATIONS IN ACCORDANCE WITH THE ATTACHED BID SCHEDULE. 3. TIME OF COMPLETION - IT IS HEREBY NOTED, TIME IS OF THE ESSENCE FOR COMPLETION OF THE PROJECT. NEW CONSTRUCTION SHALL BE COMPLETED WITHIN 45 CALENDAR DAYS OF ISSUANCE OF "NOTICE TO PROCEED". 4. WARRANTIES - THE CONTRACTOR SHALL WARRANT ALL LABOR AND MATERIALS FOR A PERIOD OF ONE YEAR FROM DATE OF ACCEPTANCE. 5. RECORD DRAWINGS - THE ENGINEER WILL FURNISH THE CONTRACTOR WITH A SET OF BLUEPRINTS TO USE FOR PREPARA11ON OF RECORD DRAWINGS. THE CONTRACTOR IS TOTALLY RESPONSIBLE FOR THE PREPARATION OF PROJECT RECORD DRAWINGS AND THEIR ACCURACY. IF PRESENT, THE ENGINEER'S CONSTRUC11ON OBSERVER MAY ASSIST THE CONTRACTOR IN PREPARING THE RECORD DRAWINGS, BUT THIS SHALL NOT LIMIT THE CONTRACTOR'S RESPONSIBILITY FOR THE PREPARATION OR ACCURACY OF THE RECORD DRAWINGS. 6. THE GROUNDWATER AT THE TIME TIIE TEST BORINGS WERE TAKEN WAS SUFFICIENTLY BELOW THE GROUND SURFACE, SO THAT THE REQUIRED SITE PREPARA11ON COULD BE COMPLETED WITHOUT ANY ADVERSE: EFFECTS.IF HEAVY RAINFALL AND/OR A HIGH WATER TABLE OCCUR BEFORE THE EARTHWORK COMMENCES, OR DURING THE EARTHWORK OPERATION AND THE SITE PREPARA11ON CANNOT BE ACHIEVED AS SPECIFIED, THE CONTRACTOR SHALL NOTIFY THE ENGINEER AND AN EVALUATION OF THE EXISTING CONDITIONS WILL BE CONDUCTED AND THE SPECIFICATIONS REVISED ACCORDINGLY. ON -SITE INSPECTION WE RECOMMEND THAT THE SITE PREPARATION AND SUBSEQUENT EARTHWORK BE MONITORED BY A QUALIFIED SOILS TECHNICIAN. THE SOILS TECHNICIAN SHOULD BE RESPONSIBLE TO OBSERVE THE CLEARING AND COMPACTION OF THE BUILDING AND PAVEMENT AREAS, DETERMINE THE SUITABILITY OF THE FILL. AND BACKFILL_, AND TAKE REPRESENTATIVE DENSITY TESTS ON THE PREPARED SURFACE, FILL, BACKFILL., FOOTING EXCAVATION AND RELATED TESTS FOR THE PAVEMENT SECTION(S). THE CONTRACTOR IS RESPONSIBLE FOR MEETING THE SOIL TESTING REQUIREMENTS STATED HEREIN. THE CONTRACTOR IS REQUIRED TO SUBMIT LABORATORY COMPACTION TEST RESULTS TO THE ENGINEER. PAYMENT FOR SOIL TESTING WILL BE INCLUDED IN THE CONTRACTOR'S BID ITEM. NO SEPARATE PAYMENT FOR TESTING WILL BE MADE. THE ENGINEER HAS SECURED THE FALLOWING PERMITS FOR THE PROJECT: 1. D.E.R. CONSTRUCTION PERMIT THE CONTRACTOR IS RESPONSIBLE FOR OBTAINING AND PAYING FOR ALL OTHER PERMITS WHICH MAY BE REQUIRED. SUCH PERMITS SHALL BE OBTAINED BY THE CONTRACTOR PRIOR TO THE CONTRACTOR BEGINNING WORK ON THE PORTION OF THE PROJECT REQUIRING SAID PERMITS. THE OWNER'S REPRESENTATIVE IS MR. JOHN A. ROBBINS, P.E. 407/562-0787. RECORD DRAWINGS SHALL REFLECT ALL :CHANGES IN DIREC11ON AND LOCATION OF STRUCTURES, PIPING, FITTINGS, SERVICES, UTILITY CROSSINGS, EQUIPMENT, ELECTRICAL, AND MECHANICAL WORK, AS WELL AS ANY OTHER DEVIATIONS FROM THE DRAWINGS. IF LOCATION DIMENSIONS ARE GIVEN ON THE CONSTRUCTION DRAWINGS, THE CONTRACTOR SHALL CHECK THE FIELD DIMENSION AND MARK THE INDICATED DIMENSION AS CORRECT OR ELSE INDICATE THE INSTALLED DIMENSION. IN ADDITION, ALL EXISTING CONDITIONS THAT ARE NOT SHOWN ON THE DRAWINGS SHALL BE NOTED. PERMANENT OBJECTS SUCH AS STREET CENTERLINES, STRUCTURES, VALVES, ETC., SHALL BE USED AS REFERENCE POINTS. UPON COMPLETION OF THE PROJECT, THE CONTRACTOR SHALL SUBMIT THE RECORD DRAWINGS TO THE ENGINEER. THESE RECORD DRAWINGS MUST BE SUBMITTED TO THE ENGINEER BEFORE FINAL PAYMENT WILL BE RELEASED. METHODS OF PAYMENT AND PAYMENT REQUIREMENTS - THE CONTRACTOR IS EXPECTED TO COMPLETE THIS CONSTRUCTION WITHIN 45 DAYS OF ISSUANCE OF 'NOTICE TO PROCEED' AND THEREFORE IT IS RECOMMENDED THAT ONE PAY REQUEST BE SUBMITTED UPON COMPLETION OF THE JOB. FULL PAYMENT WILL BE MADE WITHIN 10 CALENDAR DAYS PROVIDED ALL WORK HAS BEEN ACCEPTED BY THE ENGINEER. IF ALL WORK HAS NOT BEEN ACCEPTED, A 10% RETAINAGE WILL BE IMPLEMENTED AND A 90% PAYMENT WILL BE MADE, RELEASE OF FINAL RETAINAGE SHALL OCCUR UPON ACCEPTANCE OF ALL WORK BY THE ENGINEER. IT IS SPECIFICALLY NOTED, THE CONTRACTOR WILL BE REQUIRED TO FURNISH TO THE ENGINEER AN EXECUTED WAIVER/PARTIAL WAIVER OF LIEN FROM THE CONTRACTOR AND SUBCONTRACTORS AND SUPPLIERS PRIOR TO RELEASE OF ANY PAYMENT BY THE OWNER.
