The purpose of this letter is to address, and correct the units listed in my certification letter of 4-20-11
as follows. Unit 1202" may have been a mis-type. Unit #1106 was not listed for some reason.
I have reviewed the inspection drawing sheets this morning. The following units were inspected and
satisfactorily completed, units 201 through 801'(stack 01), and units 206 through 11,06.(stack 06).
This work was performed, satisfactorily completed, and- inspected from September 2610 through
February 2011. My final walk-thru and certification letter were on the same day, 4-20-11
• Units 901, 1001, and 1101 were certified under a previous permit. ok
As I am informed by the Miramar letter of yesterday, units may have been mis-numbered on the
permit. That, letter and my letter of today should clarify the actual units permitted and completed.

Pursuant to our telephone call of April 2, 2012 and your letter dated March
7, 2012, 1 contacted Michael Roberts, owner of Special Forces, the GC we
contracted to perform the concrete restoration project at The Miramar. He
informed me that he would take care of the situation. I have been in
contact with Michael Roberts no less than twice a week since mid -March in
an attempt to conclude this situation.
Over the past weekend (March 31 /April 1) 1 reviewed the paperwork
myself. I found that Permit #1004-0072 incorrectly identifies the unit
numbers being worked on; in some cases the unit numbers do not exist. It
appears that when the original permit was filed the majority of unit
numbers were inverted, 602 vs. 206, 603 vs. 306, 609 vs. 906, 610 vs.
1006, etc. Briefly, in the order as they appear on the Permit, units #602,
603, 604, and 605 are inside units and not part of the restoration project.
One unit, #606, is in the south end stack and has been restored and final
inspected. (Units #607, 608, 609, 610, and 611 do not exist.)
The unit numbers for the south end stack, #206, 306, 406, 506, 606, 706,
806, 906, 1006, and 1106, were restored and final inspected.
The inspection reports submitted to your office may seem a bit ambiguous
due to the aforementioned numbering conflict.
I spoke with John 'D. Breitenbach, P.E., the Threshold Engineer (Inspector)
on this project. Mr. Breitenbach will produce an additional report indicating
all units in the south end stack have been restored and inspected and
deemed satisfactory to the specifications prepared by CMS Engineers,
LLC, the Engineer of Record.
I believe this is The Miramar's only open building permit. If there are any
other open permits in the north end stack or any other place, please let me
know as soon as possible. I request this, as Mr. Breitenbach was the
Threshold Engineer for both the north as well as the south end stacks. If
additional documentation is required from Mr. Breitenbach, I would like to
do this all at once.
Insofar as the deadline of April 9t I is. concerned, again I am working with
Special Forces who is working with your Permitting Dept. to remedy this. I
am not as yet sure how- long this will take to conclude. I would appreciate
a 60-day extension on this matter. To make this request more reasonable,
Mr. Breitenbach would issue a letter stating that both end stacks have
been inspected to conform to all specifications. I feel confident that I could
get this letter from Mr. Breitenbach; this week.
Please confirm receipt of this letter, either by e-mail (KTTLocka-aol.com) or
telephone, (772.229.2429 or 772.631.2343) and to whether the letter re-
confirming that the #6 stack was completed is all that is required from Mr.
Breitenbach.
I will be working closely with Michael Roberts 'to assure that the permitting
requirements are met as soon as ,possible.

This letter is to inform you that threshold building special inspector, John Breitenbach PE 59770.
will be performing building Threshold Inspection services for the upcoming structural restoration
project at the above referenced location. You should have received project specifications from the
general contractor, Special Forces, which were prepared by CSM Engineers, LLC, the Engineer of
Record.
My threshold building special inspector number is 2072: firm registration #26001. Additionally, the
following will be authorized by me to perform inspections: Judy Perkins, PE 62332 and Charles
Darden, BSCE.
I will be sending you copies of inspection reports and a final inspection report when the project is
completed. The following is the inspection plan:
1. Concrete repair of balcony slabs.
2. Door buck attachment per specifications.
3. Door attachment per product approval submittals (17-units).

and site work to remain, which may be damaged or altered as a result of the
Contractor's performance of the work. Existing conditions of all building
components that are in proximity of the Work shall be surveyed and documented
by the Contractor prior to commencement of work.
2. Removal of Existing Accordion Shutters as Required:
a) Shutters shall be removed from the balconies, stored, and then replaced per
original construction.
3. Balcony Surface Preparations:
a) Remove and dispose existing; tile and other floor finishes, including adhesives that
may exist beneath floor coverings.
b) Prepare concrete surface to achieve proper adhesion of a waterproof traffic
coating.
4. Concrete Repairs at Balconies and Walkways
a) Demolition, surface preparation and installation of repair materials of the
deteriorated concrete and reinforcing steel as authorized by the Engineer.
b) At balcony edge repairs, excavated areas will be repoured to match adjoining
surfaces.
c) Provide Sika Galvasshield XP+ galvanic anodes at perimeter of repaired area as
directed by the Engineer.
5. Replacement of the Balcony Sliding Glass Doors
a) Remove, dispose, and replace all of the existing sliding glass doors of units 101,
201, 301, 401, 501, 601, 701, 801, 602, 603, 604, 605, 606, 607, 608, 6010, &
6011. See door manufacturer's specifications/NOA's attached.. All other units
shall have the doors removed and reinstalled per original door manufacturer's
specifications.
b) Contractor shall provide design shop drawings, product approvals and engineering
calculations in accordance with Miami -Dade County Notice of Acceptance
(NOA) criteria, signed and sealed by a:Professional Engineer licensed in the State
of Florida if sliding glass doors are to be replaced. Follow the manufacturer's
specifications/NOA for installation instructions.
6. Installation of Balcony Waterproofing System:
a) Apply Sika Ferrogard or Deggusa Protectosil over repaired concrete deck.
b) Prepare balcony surface so that it has a positive slope. Provide repair mortar of
approximately'/4" depth under new sliding glass doors and slope to balcony edge
using Sikaquick 1000 or equal.
c) Provide Sika Sikalastic, STO Decocoat, or BASF Sonoshield waterproofing
system in accordance with manufacturer's specifications.
