SPECIAL INSPECTOR THRESHOLD BUILDING AFFIDAVIT
I, Rick V. Baldocchi, the Special Inspector, an Ehgineer certified, licensed, and registered with the State
of Florida, P.E. Registration No. 38092, and certified by the Florida* Board of Professional Engineers as a
Special Inspector, S.I.Registration No. 1019, hereby certify that I, or my duly -authorized representative,
shall perform the structural inspections for the above -referenced project in accordance with the approved
structural inspection plans prepared by the Engineer of Record for the above -referenced threshold
building. I.
Further, I hereby agree to file, as required by the Building Official, signed and sealed inspection reports
which will accurately reflect the quality and I$rogress of the structural work for the above -referenced
threshold building to the Architect / Engineer, Owner, and Building Official.
Further, upon completion of the building, and prior to the issuance of a Certification of Occupancy, I, as
the Special Inspector, shall file a signed and sealed statement with the enforcement agency, in
substantially the following form: To the best of my knowledge and belief, the construction of all structural
load -bearing components described in thel threshold inspection plan complies with the permitted
documents, and the specialty shoring design 1professlonal engineer has ascertained that the shoring and
reshoring conforms with the shoring and reshoring plans submitted to the enforcement agency.
