This Water Intrusion Survey based upon visual inspection of exposed and readily
accessible surfaces. Measurements are selective and probably do not detect all moisture
that is or was present. Materials that were previously wetted and are now dry and do
not exhibit visible vidence of water -mold damage will probably not be identified as
requiring remedial action - despite the likely presence of hidden mold damage. Water
affected areas that are relatively small or relatively dry are probably not detected by the
Survey.
The results of this ater Intrusion Survey are based upon conditions existing at the time
of inspection. Con lusions are based solely upon observations during the time period
indicated, and an understanding of prior events provided by others. Because roof leaks
and other water intrusion entry sources are not generally repaired, continuing rains can
cause continuing damage that is not present or as extensive at the time of inspection.
Inspection is condu�ted without observation into hidden spaces or spaces that are not
readily accessible. (MIore extensive and intrusive inspection would probably reveal
different informatioi and result in different conclusions. Any distinction made between
various conditions t subjective and limited to the observations and testing described
herein. Conclusio and recommendations are based upon professional judgment with no
warranty or guaran ee intended or implied.
Designations for the amount of wall material removed include direction by Condo
management. These decisions include a) removing at least four feet of wall material if
any material is removal at all and b) not removing any wall material unless visible mold
staining is observed bn the wall or behind the baseboard. The latter decision will
probably result in leaving hidden mold contamination within wall spaces, behind mirror
panels, and behind cabinets that are suspected of incurring water intrusion but do not
exhibit visible mold staining at the time of inspection..
The recommended remedial protocol presumes that the remediator is fully competent and
follows all generally recognized practices and precautions in addition to other
recommended measures. The protocol is designed to specify site specific measures and not
to detail general remedial practices and precautions.

additional water-tiiold damaged material is revealed, then this additionally

REMEmunoN - GENERAL
- Removal pracI ices must protect non -contaminated portions of the
condominiurr}, contents, and the air handler system. Remediation
practices should be consistent the guidelines described in the New York
City Health Department Guideline on Assessment and Remediation of
Fungi in Indoor Environments
(http//www.ci.nyc.ny.us/html/doh/html/epi/moldrptl.html), the
guidelines desk ribed by the EPA - Mold Remediation in Schools and
Commercial Buildings
(httv://WWW.EDa.gov/iaq/molds/imaaes/moldremediation.lpdf), the
guidelines de+ibed by OSHA - A Brief Guide to Mold in the
Workplace (http://www.osha.gov/dts/shib/shib101003.html), and the
Institute of Inspection, Cleaning, and Restoration Standard and
Reference Guide for Professional Remediation S520. In cases where
guidelines do ;ot agree, the most stringent guideline should be followed.
- In cases in which significant ceiling damage exists, wall materials contiguous
with the damaged ceiling are recommended generally for removal from .
floor -to -ceiling despite the absence of visible water -mold staining or
measurable wetness. This measure is based upon the presumption that
water intrusion from the ceiling area probably resulted in the flow of water
downward"and to some degree throughout the connected wall spaces.
- In all cases, the individual air handler systems must be cleaned and
sanitized. This cleaning can be limited to the air handler cabinet, coils, and
return plenum - unless the air ductwork is identified as water -mold
contaminated, in which case the ductwork must be also cleaned and
sanitized.

In all cases that water -mold damage is identified in the vicinity of kitchen
counter cabinets, or bathroom vanity cabinets - and similar built-in
components L the cabinets must either be removed or opened to allow
inspection of the hidden surfaces. If these exposed surfaces are water -mold
damaged, then the damaged materials must be removed. If the backsides
and undersides of the cabinets are water -mold damaged, then these
damaged surfaces must be cleaned; or, if cleaning is not feasible, the
cabinets mus be discarded. Likewise, surfaces hidden by mirrors, or similar
panels, in the vicinity of water -mold damage must be removed to allow
inspection of idden wall surfaces. Again, any revealed water -mold
damaged mat, rial must be removed.
It is critical that the remediator remove all visible water -mold damaged
materials. As materials designated in this report are removed, the
remediator must inspect remaining surfaces to identify further damage that
was not evident t during original inspection or has ensued with the passage of
time.

3) Humidity Control. Relative humidity in the work zone and all other areas
of the Condo rust be controlled consistently below 60% by means of

operating portable dehumidifiers. The air conditioning system may be used
for climate control outside the work zone(s) - provided that the system is
fully isolated Iform the work zone(s).
4) Surfaces and Contents. Contents and surfaces in the vicinity of the
damaged areas must be cleaned based upon the presumption that
particulate cointamination may have settled on the exposed surfaces.
Cleaning of these surfaces should include HEPA vacuuming and, where
feasible, wipii g with a soap solution. Measures should be taken to protect
the cleaned a Ifid covered articles from 1) humidity over 60% and 2)
contamination from other spaces within the Condo.

order to av �id interference with Clearance Testing, it is suggested that
refrigerator, be isolated from the work zone(s) and other portions of the

should include the following steps:
a) Removing all materials as indicated on the attached univby--unit floor

be fully removed. If the remediator believes that such additional removal
exceeds his scope of work, then the issue should be immediately raised with
the client.

using HEPA vacuuming and, wherever feasible, wiping with a soap
solution.
d) Allow the v all spaces and floor to dry thoroughly.
e) Cover and 'seal exterior wall openings, openings into ceiling spaces,

surfacls are the basis for a failed Clearance Test.

remediati n steps are about to be completed. Any lead-time provided
will facilit to a more rapid turnaround of sampling and analysis. This
inspection must be completed before closing wall spaces with
replacement materials. The purpose of this inspection and testing is to
determine that the environment within the work zone is restored to
normal conditions with respect to mold contamination.
All cleaning activity and operation of HEPA filters must be discontinued at
least 24 hours preceding Clearance inspection/testing. Relative humidity
must be maintained below 60% until the remediation is approved as
complete based upon Clearance Testing. Operation of HEPA filters at the
time of 0earance Testing is basis for failing the Clearance Inspection.
Build back or replacement of removed materials associated with this
remed4ion must not proceed until the remediation is approved as complete
based upon Clearance Testing. Exposed and cleaned surfaces must remain
fully exposed for visual inspection and testing during Clearance Testing. All
isolation) barriers, including the work zone barrier(s), must remain in place
until approval is received based upon Clearance Testing.

(29CFR 1916.134), as well as gloves, and eye protection.
✓ Work Area. Any openings to the work area should be covered with plastic
sheets and taped before remediation begins to contain dust and debris. The
work area should be unoccupied except for the work crew.
✓ Dust Suppression. Mold -affected materials should be cut and removed in a
manner that minimizes dust creation. Dust suppression methods (example:
light misting ith surfactant -sanitizing solution) should be used wherever
practical.
✓ Removal. Contaminated materials that cannot be cleaned should be
removed from the Condo in sealed bags or taped plastic sheets. There are
no special requirements for disposal of these materials.
