THE J NDERSIGNED HEREBY REQUEST RELEASE OF ELECTRICAL POWER TO THE ABOVE DESCRIBED
PRO �iERTY, FOR A PERIOD NOT,TO EXCEED THIRTY (30) DAYS, FOR THE PURPOSE OF TESTING SYSTEMS
AND, QUIPMENT IN PREPARATION FOR A FINAL INSPECTION. IN CONSIDERATION OF APPROVAL OF THE
REQ EST WE HEREBY ACKNOWLEDGE AND AGREE AS FOLLOWS:
1. This temporary power release is requested for the above stated purpose only, and there will be no
occupancy of any type, other than that permitted by construction during this time period.
2. As witness by our signatures, we hereby agree to abide by all terms and conditions of this agreement,
including Building Division Policy, which is incorporated herein by reference.
3. All conditions and requirements listed in'the attached document entitled `Requirements for 30 Day
Power for Testing" have been fulfilled and the premise is ready for compliance inspection.
4. All requests for an extension beyond 30 days must be made in writing to the Building Official stating
the reason for the request. Power may be removed from the site and/or a Stop Work Order issued if
the Final Inspection has not been approved within 30 days. A fee of $100.00 will be required to lift
the Stop Work Order.
WE H REBY RELEASE AND AGREE TO HOLD HARMLESS, ST. LUCIE COUNTY, AND THEIR EMPLOYEES FROM
ALL BILITIES AND CLAIMS OF ANY TYPE OF NATURE WHICH MAY ARISE NOW OR IN THE FUTURE OUT
OFT', IS TRANSACTION, INCLUDING ANY DAMAGE WHICH MAY BE INCURRED DUE TO THE
DISC NNECTION OF ELECTRICAL POWER IN THE EVENT OF VIOLATION OF THIS AGREEMENT.

SUBJECT: HOUSE REPAIR WIRE LATHE REVISION CERTIFICATION

FIRE DAMAGED HOUSE REPAIR LOCATED Cam:

BACKGROUND:
THE REFERENCED PERMIT WAS ISSUED BASED ON THE APPROVED PLANS FROM THIS
ENGINEER OF RECORD FOR THE RESTORATION OF A FIRE DAMAGED RESIDENTIAL
HOUSE AT THE ABOVE ADDRESS.
THE PURPOSE OF THIS LETTER IS TO CERTIFY THE PLANNED REVISION IN WHICH
THE ASPHALT PAPER -BACKED WIRE LATHE WILL BE INSTALLED WITH (2) LAYERS IN
PREP FOR A STUCCO EXTERIOR FINISH OVER THE 2ND LAYER.
SUMMARY OF THE PLANNED REVISION IS AS FOLLOW:
• THE 15T LAYER OF 2.5 ASPHALT PAPER -BACKED GALVANIZED STEEL WIRE
LATHE WILL BE ATTACHED TO T-111 OR EQ WOOD SIDING USING
GALVANIZED 14" MIN RINGSHANK NAILS INTO WOOD STUDS AT 16" ON
CENTER.
• A 2ND LAYER OF ASPHALT PAPER -BACKED GALVANIZED STEEL WIRE
LATHE WILL BE ATTACHED THRU THE 1ST LAYER INTO T-111 OR EQ WOOD
SIDING USING 1T GALVANIZED RINGSHANK NAILS INTO WOOD STUDS
AT 16" ON CENTER IN PREP FOR STUCCO EXTERIOR FINISH PER PLAN
SPECS.
THIS PLANNED REVISION FOR A DOUBLE LAYER OF WIRE LATHE AS SPECIFIED
ABOVE WILL MEET OR EXCEED THE ORIGINAL PLAN SPECIFICATIONS FOR A 160 MPH
(VULT), EXPOSURE W APPLICATION (2017 FLORIDA BUILDING CODE) AND IS
THEREFORE AN ACCEPTABLE REVISION.
