After the Fact Re -roof Permit for Main Residence and Guest House Located at 12863
South Indian River Drive, Jensen Beach, FL 34957
presentative from Mathers Engineering Corporation visited the above referenced residence to observe the
iously installed reroof. The following observations were made:
• Dry -in nailing was verified by visual observation of the underside of the existing sheathing. Dry -in
nails could be verified visually. Areas observed were made accessible through an attic access port in the
garage. Dry -in was one layer of PolyGlass USA, Inc.'s Polystick Underlayment (Miami -Dade NOA
#17-0614.22) was attached to the existing plywood sheathing
• Extreme Metal Fabricators, LLC "5V Crimp" Metal Roof System was installed in 2010. Metal roof
anchoring as per the attached Florida Product Approval (FL #17022.1) was spaced at approximately 12-
inches O.C. both at panel's edge seams and mid -seams. Metal roof panels were anchored at 6 inches
O.C. at roof outside edge and 12 inches in the field as per the requirements of the Florida Product
Approval FL 17022.1.
• Drip edge: Drip edge is 2" x 2" galvanized aluminum flashing. As a spot check, areas were lifted to
inspect the flashing. The edge flashing and seam attachments appeared to be in compliance with the
requirements of FL 17022.1.
