The work write-up/bid form is a general outline of the work to be performed. Please refer to the project specification booklet for detailed instructions for each item noted below. All items must be cost itemized in the space provided, each page of the bid documents that require signature must be signed by the contractor and received before the deadline or the bid will be rejected. Item # Location Description of Work Price 1 HVAC ' Correctly support and connect AC ductwork in attic refer to pages 16 $ & 17 of the inspection report. • Correct the electrical overhead supply conductors a.k:a. entrance cable to a minimum of 8 ft. above the roof surface of the 31h pitch roof • Install an up to code dryer vent • Change out mismatched circuit breakers at the panel box in the mechanical closet and check the oven breaker for correct size - refer to page 24 of the inspection report, top right picture • Check all GFCI outlets in wet areas & exterior, including the rear 2 Electrical enclosed patio - refer to page 25 of the inspection report $ • Check wiring in the attic: repair any wiring that is not done correctly and seal all openings around the top framing/wall track - refer to page 26 of the inspection report • Install a GFCI receptacle and a resistant cover over the outlet in the yard under the control box for the lift station for the waste system at the east side of the property refer to page 26 of the inspection report • Install vent fans in both baths. • Replace plumbing vent stacks on the roof • Replace cover over PVC plastic plumbing drain line for the septic tank - refer to pages 22 and 23 of the inspection report 3 Plumbing . Replace toilet in west bath with an ADA toilet. $ • East Bathroom - Replace tub in the east bathroom with a tiled shower, replace sink and faucet, vanity to be provided by selected cabinet supplier. Replace toilet with an ADA toilet 4 Painting Repair and paint, if needed, any areas that have been disturbed by your $ work. Match as closely as possible. Replace all exterior doors that aren't hurricane impact with hurricane impact 5 Doors ADA doors. All locks to be keyed alike. Replace the framing around the front $ entry door - refer to page 13 of the Inspection Report AM, I 1 6 Insulation Securely fasten all fiberglass batt insulation that is loose in the attic �® i V 0 ®®V� g� of 10 A •1 ITS 2019-2 • Replace the vanities in west bathroom - refer to page 27 of the inspection report 7 Cabinets & • Replace the doors and draw fronts ONLY (with wood) of the kitchen vanities cabinets and provide door/drawer pulls. • Replace vanity in east bathroom - refer to page 28 of the inspection report 7 Appliance Install a new black range and vented microwave over the range - refer to spec book for requirements 8 Windows Replace the window in the east bathroom. $ Driveway Install paved driveway and parking on the west side of the -building 10 & parking - refer to page 25 of the inspection report Special guidance should be sought from Planning and Development Services. ADA complaint sidewalk should connect from the house to the parking area. • Trim overhanging branches and trees away from the roof • Install gutters and downspouts around the entire perimeter of the building. • Repair/Seal cracks & openings around penetrations in exterior walls - refer to page 10 of the inspection report • Replace door hinges - refer to page 20 of the inspection report • Remove and replace fence with shadowbox style fence around the house - refer to pages 29 & 30 of the inspection report $ 11 Misc.. . Retile the east bathroom floor. TOTAL PROJECT COST S *Prices shall remain firm for a period of 90 days. Award of the bid is contingent upon clearance of the Environmental Review Record and recertification of the applicant's eligibility. * NO QUALIFICATIONS, COMMENTS OR ADDITIONS PERMITTED ON THIS WORK WRITE-UP.
