the neessary service to protect the above property against subterranean termites (Non -Formosan).
YOUR LIMITED GUARANTEE
THIS LIMITED GUARANTEE IS ARETREATMENT ONLY GUARANTEE. In consideration of sums received by
us for treating the above premises for Subterranean termites, we guarantee to inspect annually the above premises and to
apply any necessary liquid barrier treatment to said premises if Subterranean Termite infestation is found during the period
that this Limited Guarantee remains in force.
TERMS, CONDITIONS AND EXCLUSIONS
THE INITIAL PERIOD OF THE LIMITED GUARANTEE SHALL BE ONE YEAR, COMMENCING ON THE
DATE OF THE INITIAL TREATMENT.
Patrick Exterminating, Inc. will extend protection annually to the owner of the identified property subject to the general
conditions of this agreement for $ N/A per year payable on or before the end of the previous
protection period. After the fifth year and each year thereafter, Patrick Exterminating, Inc. reserves the right to revise the
annual',renewal charge.
Patrick Exterminating, Inc. shall not be liable for retreatment to covered premises that have been added to or structurally
alteredunless Patrick Exterminating, Inc. has been notified prior to the beginning of any alteration or addition, and any
additioi I ial treatment or retreatment Patrick Exterminating, Inc. finds necessary is performed. Such alterations and
additions must be serviced by Patrick Exterminating, Inc. as soon as practical - at the expense of the owner. Additions or
alterations include the addition or removal of soil around the foundation. This agreement excludes Formosan Termites.
The presence of water in treated areas or through interior\roof leaks are conducive to Subterranean Termite infestation.
Purchaser is responsible for making timely repairs or modifications where conducive or alternate water sources are found.
Upon completion of repairs by Purchaser, Patrick Exterminating will provide, if necessary, -additional treatment to control
infestation at Purchaser's expense. If purchaser elects to not repair said defects then Patrick Exterminating shall have no
fin-ther obligation under this agreement. Patrick -Exterminating, Ina --requires access to the -covered premises or -any
additions for the purpose of carrying out the terms and conditions of this agreement. Customer agrees to make premises'
accessible for annual inspection.
This protection plan will be effective only upon payment of the charges provided herein.
