Yes, we are still waiting on a revised site plan for this location. It will need to have the correct OSTDS and well locations
as well as a 75' well setback arc around the well.
The well, should not be located in the driveway as it is required to protrude 12" above grade.
We will not give final approval until we receive the as -built and have inspected the well. If the well moves any
substantial distance, we may require the well permit to be revised.

Hey Lydia,
Yes, having the well in the middle of a gravel driveway would be problematic. The well head must extend a minimum of
12" above grade. We are currently waiting for an updated site plan for this location. Is that something you could send
me as a 'pdf? Id like to see where the OSTDS and well are proposed to be vs where they installed.

Good afternoon Brian,
I have a ;question. We are working on a revision to a driveway for the new house at above address. It looks like the
proposed well is in the middle of the gravel driveway. The original survey didn't indicate the existing gravel drive. Isn't
that creating a problem with the well? Please advise.

Yes, they do have a permit for the new connection. The existing connection will be used on the south end of the circular

Jimmy, '
Above permit is for a replacement home at above address. Did they ever submit a driveway permit? They are
submitting a revision to their driveway on the property creating a circular driveway. However, originally they did not
indicate) an existing driveway only a proposed. The revision now indicates an existing driveway and the proposed in a
different location. See attachment.
