Comment #03/24/2009 21, Page 2:
Englert series 1300 aluminum metal panel roofing
submitted requires a fire barrier board per system A-1,
NOA #05-033 1.01.
Answer: As per attached Florida Approvals the Fire Barrier Board is
not necessary. This test only applied to Dade County and is
not applicable to the rest of the State. See attached
Evaluation Report, Certificate of Independence for
Evaluation and Installation Method. See attachment 'C'.

deck type application, meeting the design pressure on sheet

Comment #03/25/2009 22, Page 3:
The generator is not meeting code. Minimum distance to a
building is 10'. If there are no windows and doors on the
side of the building where the generator is located and the
exhaust is not going into any vents, the distance can be
reduced to 5'.
Answer: See Attachment '13' and revised sheet A- I for exact
location and dimensions.
Comment H03/25/2009 23, Page 3:
The generator requires a separate permit.
Answer: Issue will be handled by Owner and General Contractor.

The name on the tank and piping plan still needs to be
revised.
Answer: Issue will be handled by company in charge.

Calculations have been made for each individual situation

on C-Store as required under 1404.2. 1.

Comment #07/18/2008 3, Page 3:
We need a sub summery and sub agreements for electrical,
plumbing, mechanical and roof.
The roof sub requires a roof contractor GC is not allowed
to do metal roof.
8/22/08 they will bring it in before pick up, they don't
know yet who the subs are going to be.
2/11/09 will submit subs when ready for pick up.
Answer: Will be provided by General Contractor.
Comment #01/23/2009 16, Page 3:
E-mail from Yvette, ERD, site plan extension approved,
still working on their issues with the scrubs jays, need to be
resolved before they can clear the site.
Answer: Issues will be handled by Owner and General Contractor.
Comment #02/11/2009 18, Page 3:
2/11/09 the submitted page in not having the same as the
site plan and the rest of the building set.
Answer: Architectural site plan has been changes to agree with Civil
engineer's plan which was the Site Plan Review Plan.
Generator has been moved. See architectural plan. When
site plan was approved the Generator was not shown.
Comment 902/11/2009 19, Page 3:

The pages SP-1, A- I A and A-7 still need to be revised
showing the correct location of parking space 1, the
location of the air vacuum and the location of the tank slab.
All the pages in the building set need to show the same
information.
Please submit from each page 3 signed and sealed corrected
pages by the engineer and/ or architect of records.
Answer: Architectural plans SP- I and A-7 have been changed. The
underground petroleum tanks as shown on A- I A are
required to be positioned as shown in order for the tanker
truck to drop their load. This is only done'off of the rear,
passenger, side of the truck.
Comment #02/16/2009 20, Page 4:
License from the General Contractor is still on hold. We
updated the state license, but the fees for the county cert.
still need to be paid. License is still on hold.
Answer: Will be done by the Contractor.
Comment #02/17/2009 21, Page 4:
2/16/09 new comments to contractor by fax and owner by
mail.
2/17/09 forwarded to plan review.
Need to return to zoning for ERD and storm water check.
Plan review fee needs to be adjusted.
