Per your request, Florida Engineering & Testing Inc. has reviewed client provided in -progress photographs
and a client provided roofer affidavit/letter, dated March 2, 2019, to verify the re -roofing workmanship and
materials used to determine if underlayment installation including metal flashings was completed to meet
the minimum standards of Florida Product Approval # FL 16048-R6, the manufacturer's installation
instructions, and the 2017 Florida Building Code. The following is a summary of our observations of the
roof in -progress installation:

To whom it may concern, at the above address we Fl Contract Services tore off the existing shingle
roof covering down to the 5/8" CDX plywood, re -nailed the decking with 8d ring shank nails six (6)
inches on center throughout, hammered down any nail heads that weren't flush with the plywood
decking, installed one layer of Tri-Built Smooth HT S/A underlayment back nailed twelve (12)
inches on center, installed 24-gauge galvalume valley flashing and bullnose drip edge nailed 6" O.0
with 1-1/4" ring shank nails. We then rolled a 401b metal roller over the entire roof covering to
adhere the new underlayment to the plywood substrate.
