We are in receipt of your Request for Approval to Place a Water Distribution System into Operation,
for the project authorized by the above referenced permit number that the subject water main distribution
system extension has been constructed, tested, and disinfected In accordance with the plans and related
materials permitted by this agency, and is now ready for operation.
Based on your engineer's certification of construction completion, satisfactory bacteriological results, and the
pressure & leakage test report for the above -referenced project, the project is hereby released for service in
substantial accordance with the approved plans with the understanding that the project will be operated in
compliance with Florida Administrative Code (F.A.C.) Chapter 62-604.
Approval of individual connections to a completed distribution system must be obtained from the City of Port St
Lucie Utility Systems Department, separate of this notice by a Letter of Acceptance by the Utility Director or his
designee.
