This letter is to inform you that you are approved to proceed with construction of the screened
smoking room at Rio Arcade located at 6649 S. US Highway 1, Port St. Lucie, Florida.
This approval is based upon the inspection conducted on May 12 and the final plans submitted
to the Department of Health (Department) on May 18 with the agreement that this room will be
more than 50% opened around its sides by surfaces (screens) that do not block
You will be required to notify the Department upon completion of the construction for final
review. An inspection will be conducted by the Department to verify compliance.

In a re -review of the revised plans which were submitted to the fire dept 04/04/2011 it is noticed that the revised
plans still specify that the build -out will still be sprinklered in the project description. I contacted Mr. Ray Chladny the
architect and questioned his calculations of the occupant loads as they differ from what is required by fbc code table
1004.1.1. He stated to me that these were given to him in a meeting with a code professional with Poma and he really
did not want to discuss it. I told him what the description stated in the code for " gross area " and he agreed but again
did not wish to discuss it and stated it would be on us to make any determinations. This is info for your determination of
possibly a higher review by either the Board of Architects or DCA. I have loaded the fire sprinkler inspections as directed
by the plans until a further notice.

SLC reduced the permit fees on Harbor to reflect our contract amount.
I have a copy of the executed lease for Rio. I am sure that Marcos signed the tenant improvement scope of work;
however, it doesn't appear in my scanned copy. Please send me an image of the signed SOW so that I can get the permit
fees reduced on this one too.

Dear Mr. Cicio,
Please allow this correspondence to serve as notice that there shall be no roofing related work on
the two permits above. There exists one 2-6 ton RTU curb per unit ( 9 total) that were installed
during the construction of the Kitterman Plaza (Shell Permit: #SLC-0707-0033). We designed all
HVAC systems within the buildouts to utilize these existing curbs. Please remove the
requirement for Subcontractor roof verification from these permits so that I can schedule several
inspections for Harbor Insurance on Monday morning. Please don't hesitate to contact me with
an questions or concerns related to the above. Thank you in advance for your cooperation.

A. Underground plumbing lines for bathrooms and kitchen.
B. Electrical floor receptacles for game aisles.

Framing and Drywall
A. As per preliminary plans.
B.' Exterior walls to be 518" drywall over 1.5" Hat Channel @ 16" on center.
C. Newly constructed interior walls to be 5/8" drywall over 3 5/8" or 6" metal studs as per plans.
D. All hard ceilings to be rh" drywall over 3 5/8" metal framing.
E. 'h" Drywall over metal framed soffits at cashier and perimeter of game rooms as per plans.

Insulation
A. R-5.1 Fi-foil insulation at all exterior walls.
B. R-11 fiberglass batten insulation (sound proofing) around all bathroom walls and walls containing
plumbing drain lines.
Plumbing
A. Plumbing as per plans and specs.
B. Automatic valves shall be installed in public bathrooms only for all lavatories, Urinals and Water
Closets.
C. Standard valves to be utilized at upstairs bathroom and unisex employee bathroom within locker
room.
D. 80 Gallon electric HWH installed within supply closet.
E. Kitchen plumbing tied into grease interceptor as required.
F. No drain line to be provided at fountain. Water will have to be pumped out.
6. HVAC
A. New RTU package units and duct board/flex duct distribution system as per plans and specs.
B. Root top exhaust fans interlocked with that room's HVAC systems as per plans.
C. Bathroom and Foyer exhaust fans as per plans and specs.
D. Smoke -Eaters are not included in this scope.
E. No grease hoods or heat hoods are included within this scope.
7. Electrical
A. Electrical service, sub'panels, lighting, emergency/exit lighting and fixtur r plans and specs. s pe
B. Wall data drops to consist of 4x4 galvanized metal boxes with r/4" conduit and pull string stubbed
into accessible ceiling (one per office).
C. Floor data boxes shall be. installed adjacent to floor receptacles.
D. (1) Cat 5e home run shall be installed at each box and terminate upstairs within server closet.
Patch panels and termination of wire ends shall be by others.
E. Phone/data and POS systems are by others.
F. Audio Visual systems are by others.
G. Security System is by others..
8. Fire Sprinklers
A. Design, engineer, permitting, construction layout, as-builts and restoration for installation of onsite
fire line and fire sprinklers.
B. Install wet tap, valve, backflow preventer, fire department connection, post indicator valve, and
fire line to building.
C. Install fire sprinkler system for A-3 Assembly use.
9. Fire Alarm
A. Design, engineer and permitting for fire alarm system.
B. Addressable system to have voice evacuation, horns, strobes, pull stations as required for A-3
Assembly use.
10. Elevator
A. Limited Use/Limited Access elevator to access the second floor.
B. Price includes a fully enclosed cab.
