Dear Mr. Cicio,
Please allow this correspondence to serve as notice that there shall be no roofing related work on
the two permits above. There exists one 2-6 ton RTU curb per unit ( 9 total) that were installed
during the construction of the Kitterman Plaza (Shell Permit: #SLC-0707-0033). We designed all
HVAC systems within the buildouts to utilize these existing curbs. Please remove the
requirement for Subcontractor roof verification from these permits so that I can schedule several
inspections for Harbor Insurance on Monday morning. Please don't hesitate to contact me with
anX questions or concerns related to the above. Thank you in advance for your cooperation.

Note: This proposal is based on preliminary layout by Jason Poma dated 9/14/2010.
Build out to include:
1. Building. Permit Fee - Excludes Impact Fees, if any.
2. Concrete
A. Concrete cutting, removal and patching for new underground plumbing lines.
3. Framing and Drywall
A. As per preliminary plans.
B. Exterior walls to be 5/8" drywall over 1.5" Hat Channel @ 16" on center.
C. Newly constructed interior walls to be 5/8" drywall over 3 5/8" metal studs with top of framing
terminating at 9'-0".
D. All hard ceilings to be %z" drywall over 3 5/8" metal framing.
E. Wood or metal backing at all upper cabinet locations, bathroom fixtures and accessories.
F. All walls to receive smooth, slick, level 4 finish. Knockdown texture is not included.
4. Insulation
A. R-5.1 Fi-foil insulation at all exterior walls.
B. R-11 fiberglass batten insulation (sound proofing) around all bathroom walls and walls containing
plumbing drain lines.
5. Plumbing
A. HC Men & Women bathrooms and water fountain with cold water supply only are existing.
B. Undergrounds, top -out plumbing and fixtures for employee break room as required.
C. Insta-hot point of use water heater to serve break room only.
D. Break room sink garbage disposal is not included.
6. HVAC
A. Units 7 & 8 HVAC Systems are existing. These systems' ductwork will be modified as required to
properly distribute air within new offices.
B. Unit 6 HVAC system to be sized by MEP Drawings.
C. Includes digital, programmable thermostats.
D. Duct smoke detector(s) and remote test station(s) are included.
E. Manual outside air dampers included.
7. Electrical
A. Electrical service, sub panels, lighting, emergency/exit lighting and fixtures, as per plans and specs.
B. Phone/Data drops to consist of 4x4 galvanized metal boxes with 3/4' conduit and pull string
stubbed into accessible ceiling (one per office).
C.
Low voltage wiring andphone/data systems are by others.

Door & Trim Installation
A.
3'-0"x6'-8"x 1 3/8" Single-lite French doors at all employee offices and conference room, paint
grade.
B.
3'-0"x6'-8"x 1 3/8" Hollow Core Masonite doors at all other enclosed rooms, paint grade.
C.
Standard 2 %d' FJ Pine colonial casing at doors.
D.
Wood base excluded.
9.
Hardware
A.
Keyed entry doors at executive offices.
B.
Passage locks at all other interior rooms.
C.
Privacy locks at bathrooms existing.
D.
Wall mounted bumper stops as required.
E.
All hardware to be US26D brushed chrome finish. Unless noted otherwise.
10.
Cabinets
& Countertops
A.
Builder's grade, knock -down, upper and lower cabinets within break room.

Preformed Formica countertop within break room.

A.
Wall paint existing within Units 7 & 8. Touched up as required.
B.
All newly constructed areas to receive primer and finish coat of the existing color.

A.

A. Professional fees, revision fees, courier costs and printing due to tenant changes after permit
application has been submitted.

PAYMENT TERMS: Deposit in the amount of ($5,075.00), to accompany executed scope of work Receipt of
deposit and this executed document constitutes authorization to proceed. A schedule of values will be submitted
for approval prior to the start of construction. Subsequent to commencement of construction, a draw request
(Application for Payment) will be submitted every four (4) weeks based on percentage of completion as applied to
the schedule of values. Payment shall be made within 15 days of application date. No retention is to be held. Any
portion of the draw request not paid by due date will incur finance charges at a rate of 1.5% per month. --

29.2 Additional Landlord I nnrovmnents: Landlord shall construct, at its expense, all other tenant
improvements agreed upon by Tennnt atd Lendlod, includbig wilhoat limitation, permitting, drawings, additional
interior finishes and Prim, flooring, fixtures, low vol(age wbi g, Rn•aishhngs, security system, cabhtets, coruterlops,
data systems, window Irenhments, workstations, appliances, etc. ("Additional Lnndlord Improvements"), Priorto
commencement of file Additional Landlord Improvements, Tenant shall deliver to Landlord its preliminary plans for,
review by Landlord, Within tell (10) days after delivery of the preliminary plans, Teunnt shell deliver to Landlord
