Re: Evaluation Report for Residential Model 5140/9100/9600 family
FL 09274-116
To Whom It May Concern:
At the request of Wayne -Dalton, a Division of Overhead Door Corporation, I have reviewed
the drawings and tests listed below. These doors were tested at an accredited laboratory
according to ANSI/DASMA 108-2002 and ANSI/DASIVIA 115-2005 test procedures. The pressures
listed on the drawings are a direct result of these tests. I have concluded that the construction
shown on these drawings comply with the structural requirements of the 2007 Florida Building
Code. I certify that I meet the requirements of "independence" as detailed in Florida Statute
913-72, section 72.110.

Wind Loads
ANSI/DASMA 115 - Windborne Debris testing
ANSI/DASMA 108 - Static windload testing
Testing was conducted in accordance with ANSI/DASMA 108-2002 and 108-2005.
ANSI/DASMA 108-2002 is forward compatible with ANSI/DASMA 108-2005. All tests included
holding the door at design pressure for a minimum of 10 seconds, and achieving a 150% overload
("test" or "proof") pressure for 10 seconds. In addition, some of the doors were also tested for
windborne debris according to ANSI/DASMA 115-2005. Not all windborne testing included glazing
with the door panels. These are itemized in Tables I and 2.

Installation Instructions
The drawings provide installation instructions peculiar to windload reinforcement. Inaddition,
Drawing 307494, Rev P10, provides supplemental track assembly details.
Jamb attachment:
Methods of securing the door to the mounting surface Is detailed in the Jamb Connection
Supplement, P/N 324620, Rev P9.
This Evaluation Report does not address design of the wallAambs themselves. These
drawings only illustrate common means to attach the door to the jambs. Walls and Jambs
should be designed (by others) to withstand the loads Imposed by door onto the building.
