THIS COURTESY NOTICE IS SENT TO YOU PRIOR TO THE EXPIRATION DATE OF YOUR PERMIT. IN DOING SO, THIS GIVES YOU THE OPPORTUNITY TO FINAL YOUR PERMIT BEFORE THE REQUIREMENT TO RENEW YOUR PERMIT OR SCHEDULE AN INSPECTION BY CALLING (866) 284-1280. PLEASE DO NOT DELAY.
