This letter is to certify that construction of the subject project has been completed in substantial
conformance with the approved plans, except as noted:
1. The car wash and the traffic separator on the north side of the car wash was not constructed.
2. The parking stalls and vacuum islands have been re -arranged on the south side.
3. The 6" step at the pavement and sidewalk interface at the east side of the store has been
partially eliminated to avoid a tripping hazard.
An as -built record drawing is attached for your records. Material test reports are on file at our
office.

incorporated herein by reference.
3. All conditions and requirements listed in the attached document entitled
"Requirements for 30 Day Power for Testing" have been fulfilled and the
premises is ready for compliance inspection.
We hereby release and agree to hold harmless, St. Lucie County, and their employees
from all liabilities and claims of any type or nature which may arise now or in the future
out of this transaction, including any damages which may be incurred due to the
disconnection of electrical power in the event of violation of this agreement.

The Rigid Insulation for the above project was installed as follows:
1) 3" Polyisocynurate (R=21) was installed over plywood deck.
Secured with screws and 3" galvalume insulation plates
and according to FM I-90 fastening pattern (enclosed).
2) 1/2" Perlite 4 x 4 sheets (R=1.32) fully adhered in hot
asphalt. Installed over ISO.
The Johns Manville Four Ply Built -Up Roof Specification 4GIS
(enclosed) was then installed.
The insulation and built-up roof was installed according to
manufacturers recommendations.

Required Revisions:
This permit review is for the construction of the mercantile occupancy only. A separate permit
is required for the installation of the underground storage tanks, piping, and the dispensers.
1. If the exterior doors marked D-1 & D-2 are sliding doors they shall meet the requirements for
a power operated doors as indicated in NFPA 101-5-2.1.9. or a side hinged door shall be
provided.
2. Provide a 3A-40 BC rated fire extinguisher.
3. Provide a kill switch for both dispenser islands. Mount in the vicinity of the check out
window.
4. The A/C automatic fan shut down requires a.listed duct type smoke detector mounted in the
supply air only of each unit. Because a fire alarm system is not required a remote test station
with a sounder shall be provided. Mount in'a normally occupied area.
