Florida Power & Light Co. has no objection to the concrete slab in the rear easement at the
above referenced address. FPL has requested that you have ttvo spare 2" pve pipes installed
and capped at each end with a minimum. of 24" cover depth prior to the slab being poured.

Siocc this is a 5-foot easement for utility use, if FPL has to cut or break this slab for
maintenance of its electric facilities, FPL will not be responsible for the cost to repair this
slab or for any other structure within this easement.
Prior to any digging you must contact Sunshine State One Call of Florida. Contact them
either by telephone tall free at 1-800-432-47 70 or by e-mail at www.callsunkino.com, forty-
eight hours in advance of construction. Sunsbine State One Call of Florida will scbedule a
locator to perform the necessary locates for you at no cost.

RE: Building a permit to put a 5" thick layer of concrete over 5 ft. section
between the foundation of my house and the seawall facing the Indian
River on Lot 392.
Dear Mr. Pistner:
Adelphia has no objection to the encroachment into easement on the property
described above. Your responsibility is to place two 2" inches conduit and also a two
1" inches conduit for a drop. You also have agreed to cover cost of the breakthrough
and restoration.

Nettles Island has reviewed your request for permission to install a concrete pad
in your five foot utility easement and is in agreement with all utilities to allow
this.
Please understand should Nettles Island have need to access the area it will be
your responsibility to absorb both the costs of removal and replacement of said
concrete.

This responds to your inquiry regarding placing concrete slab that will encroach into the
rear five foot Utility Easement on Lot 392, Nettles Island, Jensen Beach, St. Lucie
County, Florida.

BellSouth does not object to the encroachment of the concrete, provided that if BellSouth
requires access to the easement for any work on its facilities in the easement area, the
property owner will be responsible for the removal and re -installation of the
encroachment (or, at BellSouth's option, any additional costs BellSouth may incur in
connection with its work due to the encroaching structure).
To the extent there are any modifications to the structure from the five inch concrete,
fiu her approval from BellSouth would be required, and the property owner would be
responsible for any additional costs or damages to BellSouth arising from the
modifications. This letter should not be construed as non -objection to any future
modifications of or additions to this encroachment.
The property owner or the owner's contractor must call before digging in the area for
locates of facilities so as not to damage BellSouth facilities - 1-800-432-4770. Florida
law requires this. See Chapter 556, Florida Statutes. BellSouth will hold the property
owner responsible for any damages to its facilities.
