As requested, a Universal Engineering Sciences Technician was present at the above referenced project
site for the purpose of earthwork testing for the Storm structures, testing for the Slab.
While on site, the technician observed the general condition of the areas to be tested and performed 6
density test(s) for Storm structures. All test(s) were performed in accordance with the ASTM D-6938
Nuclear Gauge Method and were found to meet the project compaction requirements of 98% of the
maximum dry density per ASTM D-1557.
While on site, the technician observed the general condition of the areas to be tested and cast 3 set(s) of
4 - 4 x 8 specimen(s) for compressive strength testing. The concrete tested on this day was found to be in
accordance with the project plans and specifications with regard to batch time, slump, and temperature.
The Contractor was notified of the test results or a copy was left for their records.
This test(s) was performed without deviation from ASTM procedure.
This report shall not be reproduced, except in full, without the written approval of Universal Engineering Sciences.

site for the purpose of earthwork testing for the Storm structure, Storm pipe.
While on site, the technician observed the general condition of the areas to be tested and performed 6
density test(s) for Storm structure. All test(s) were performed in accordance with the ASTM D-6938
Nuclear Gauge Method and were found to meet the project compaction requirements of 98% of the
maximum dry density per ASTM D-1557.
While on site, the technician observed the general condition of the areas to be tested and performed 8
density test(s) for Storm pipe. All test(s) were performed in accordance with the ASTM D-6938 Nuclear
Gauge Method and were found to meet the project compaction requirements of 98% of the maximum dry
density per ASTM D-1557.
As Requested, a Universal Engineering Sciences Technician was present at the above referenced
location for the purpose of Technician was on site to observe gravel being placed in canopy footings to a
depth of 12 inches. (1 foot)

While on site, the technician observed the general condition of the areas to be tested and performed 8

with the ASTM D-6938 Nuclear Gauge Method and were found to meet the project compaction

As requested, a Universal Engineering Sciences Technician was present at the above referenced project
site for the purpose of earthwork testing for the Curbs, testing for the Gas Tank slab.
While on site, the technician observed the general condition of the areas to be tested and performed 2
density test(s) for Curbs. All test(s) were performed in accordance with the ASTM D-6938 Nuclear Gauge
Method and were found to meet the project compaction requirements of 98% of the maximum dry density
per ASTM D-1557.
While on site, the technician observed the general condition of the areas to be tested and cast 3 set(s) of
4 - 4 x 8 specimen(s) for compressive strength testing. The concrete tested on this day was found to be in
accordance with the project plans and specifications with regard to batch time, slump, and temperature.
There 2.5 hours of standby time onsite.
The Contractor was notified of the test results or a copy was left for their records.
This test(s) was performed without deviation from ASTM procedure.

As requested, a Universal Engineering Sciences Technician was present at the above referenced project
site for the purpose of earthwork testing for the 2" Water Service tap, testing for the Driveway.
While on site, the technician observed the general condition of the areas to be tested and performed 2
density test(s) for 2" Water Service tap. All test(s) were performed in accordance with the ASTM D-6938
Nuclear Gauge Method and were found to meet the project compaction requirements of 98% of the
maximum dry density per ASTM D-1557.
While on site, the technician observed the general condition of the areas to be tested and cast 2 set(s) of
4 - 4 x 8 specimen(s) for compressive strength testing. The concrete tested on this day was found to be in
accordance with the project plans and specifications with regard to batch time, slump, and temperature.
The Contractor was notified of the test results or a copy was left for their records.
This test(s) was performed without deviation from ASTM procedure.

While on site, the technician observed the general condition of the areas to be tested and performed 2
density test(s) for West entrance turning lane. All test(s) were performed in accordance with the ASTM
D-6938 Nuclear Gauge Method and were found to meet the project compaction requirements of 98% of
the maximum dry density per ASTM D-1557.
While on site, the technician observed the general condition of the areas to be tested and performed 3
density test(s) for Fire hydrant line. All test(s) were performed in accordance with the ASTM D-6938

As requested, a Universal Engineering Sciences Technician was present at the above referenced project
site for the purpose of earthwork testing for the Pipe backfill, Storm structures.
While on site, the technician observed the general condition of the areas to be tested and performed 9
density test(s) for Pipe backfill. All test(s) were performed in accordance with the ASTM D-6938 Nuclear
Gauge Method and were found to meet the project compaction requirements of 98% of the maximum dry
density per ASTM D-1557.
While on site, the technician observed the general condition of the areas to be tested and performed 9
density test(s) for Storm structures. All test(s) were performed in accordance with the ASTM D-6938
Nuclear Gauge Method and were found to meet the project compaction requirements of 98% of the
maximum dry density per ASTM D-1557.
The Contractor was notified of the test results or a copy was left for their records.
This test(s) was performed without deviation from ASTM procedure.

Dear Ms. Rudisel,
We have observed that there is.groundwater at the bottom of the footing excavations for the
canopy structure that is under construction on the above referenced site. It appears that the
water might be temporarily perched storm -water, but this assumed perched water layer is
preventing the contractor from achieving the required compaction testing in the bottom of the
footings for the canopy. Therefore, the contractor has requested an alternative to meeting the
required soil compaction specification.
In accordance with local standard practice for building footings, Universal recommends that
water -effected footings may be over -excavated to a depth of at least 12 inches beneath the
design footing bottom and then backf(Iled with open graded gravel consisting of FDOT #57 or
#67 size stone or recycled concrete. Use of this material does not require compaction testing or
dewatering well points; however, we do recommend that the gravel be 'seated' with the backhoe
bucket. Universal should observe the footing excavation prior to concrete placement to confirm
the depth of gravel. Once we observe that the footings have the rock installed, then the footings
will meet the design bearing of 2,500 psf.
