Sorry Monica, I was in LEAP class this morning.
1) Work was done without permit.
2) They applied for a building permit at 2/26/15, FDEP permit was expired 2/20/15.
3) At 4/14/15 the Plans examiner's comments where completed except for the FDEP renewal permit. Contractor
was supposed to request a renewal in November 2015. Since this is an after the fact permit, FDEP will not renew
the permit.

Permit 1502-0384 is still pending. See response of Jason Spanier from FDEP. Are we going to issue the
permit since the dune cross over and fence is already there? FDEP will not be able to re -issue their
permit. Final inspection will need to be done by Engineer letter per Ed.

Hope you are doing well. I've talked to a Mr. Gary Mc Peek a few times in regards to the completed
dune walkover for the Tarpon Flats development next to the Hibiscus Condo. I'm attaching the DEP field
permit I issued in 2014 for the structure which I'm not sure you had a copy of. During that time, I
actually issued it to the agent on record (Bill Stoddard) and approved the design at our on -site meeting.
Those plans are also attached. Since this work has been completed and permit since expired, I
technically can't re -issue anything after -the -fact. If you need anything further or if I can help clarify,
please just let me know.

Enclosed please find the following information which shall constitute a request for Final Certification and
Acceptance of the paving and drainage improvements at the Tarpon Flats project, formerly known as Seaside/Heron
Cay_
• Two (2) copies of the revised Paving & Drainage Record Drawings
• Two (2) copies of the Geotechnical Testing Report prepared by GFA International, Inc.
This is office would hereby request a final inspection of the project for the purposes of providing final certification
in conjunction with the review and approval of the proposed Record Plat for Tarpon Flats. It is the intent of the
current applicant to re -surface the asphalt layer in order to provide a consistent finish to the roadways moving
forward. It is undecided at this point as to whether the current surface will be milled or completely removed and
replaced.

Plans Examiner Review 3/9/2015
1. Geotechnical report not required. Minimum embedment specified on the plans and in
the structural notes.
2. 2010 Florida Building Code and ASCE 7-10 are specified in the structural notes.
3. Risk Category I, corresponding ultimate wind speed 150 mph.
4. Exposure category D has been added to the structural notes.
5. Attached please find a copy of the FDEP field permit that was issued in October 2014.

All- I do not have an issue with the dune crossover, since it is consistent with the approved site plan. There wasn't
anything out there that was going to require mitigation and the applicant previously submitted an Environmental Impact
Report.
They will be required to fill out an after the fact vegetation removal permit application, when they apply for the after the
fact building permit. Since everything is constructed it seems like it will be a paperwork process to resolve the situation.

Scott,
The fence and the dune cross over East of the Coastal Construction Control Line requires a separate permit, as I
informed you yesterday. We are notable to add this to the current permit. Permit 1410-0021 was issued for the gate
and fence until CCCL only. Please provide an application with two sets of signed and sealed readable plans for the dune
cross, two copies of the site plan and a readable copy of the FDEP permit.
