In response to Review Comments (Documents Missing):
1. Per comment #2 on 7/2/15 ... there is no plumber on this project. Electrician should have submitted his
affidavit by now. Do you need a copy of the contract between Helmet House Construction and the
electrician in addition to the affidavit?
2. Per comment #4 on 7/13/15 ... do you need the 2 digital plan sets now or once we have made any
changes you and/or the plans examiner may request?

I met with Mr. Heffelfinger, the contractor who has been contracted by TC Hospice to develop a "Butterfly Garden" at
their facility located at the subject address. The improvements for the Butterfly Garden will not require an adjustment
to the site plan with the Planning Division. In the future should they propose improvements that include vertical
construction with enclosed buildings the adjustment to the site plan at that time will indicate the Butterfly
Garden. Please let me know if you have any questions. Thank you.
