Morrison Hershfield has completed a post construction visual observation of the above referenced project. The
scope of work under permit # SLC 1510-0031 involved the replacement and addition of antennas to an existing
monopole tower and new remote radios on the ground.
This evaluation makes use of digital photos of the as -built conditions taken after construction. Based on
observations, we have found that the new remote radios were installed on new unistrut supported by the existing
ice bridge posts as opposed to installing a single post to mount all the remote radios on which was depicted in the

The approved permit plans do not represent the as -built condition of the new remote radio installation, but this
office approves the deviation to the plans.
To avoid any misunderstanding, I hereby state that, to the best of my knowledge, belief and professional
judgment, this report represents an accurate appraisal of T-Mobile's antenna and equipment based upon careful
evaluation of observed conditions to the extent reasonably possible.
