OBSERVATIONS: - Onsite 10/21/15 with Glasgow Project Manager Jason De Gaglia for installation of doublewall aboveground storage tank (AST) Tank #21. - Tank #21 is a new Modern Welding Fireguard (EQ-352) shop -fabricated dw 5K gallon compartmented AST (2,500 ULgasoline/2,500 vehicular diesel). Outerwall breach of integrity, as stated in attached specs and onsite, was factory tested and then filled with lightweight insulating concrete, specs attached in FIRST. Tank has (2) primary fuel vents, (2) emergency fuel vents and (1) interstitial vent. - Tank #21 was installed on existing slab in exact location of closed and removed Tank #14 and Tank #15. Well siting for no public potable wells in 500 ft was provided and attached in FIRST. - Tank #21 east compartment contents will be UL gasoline and the west compartment contents diesel, color- coding/labeling are per API. - Piping is aboveground singlewall no contact with soil. Tight fills are in one fill spill shop -fabricated and tested containment on west end of the Tank #21, piping runs directly to top of tank. Dispensing suction piping to deliver fuel to separate free-standing dispensers. Glasgow installed previously existing (2) dispensers, pans and the piping above the pressure regulator/shear valves that had been used with the removed Tank #14 and Tank #15 systems. No obvious integrity issues had been noted for the dispenser pans. The pressure regulator/shear valves are noted per Jason as 52 valves. New dispensing piping is installed from Tank #21 to - Outerwall monitoring port is on the east end on top of Tank #21. - An updated storage tank registration form was emailed to Tallahassee showing Tank #14 and Tank #15 removed from site. OBSERVATIONS: - Onsite 10/1/15 with Glasgow Project Manager Jason De Gaglia and Envirospec Environmental Consultant Cindi Brunot for closure inspection of aboveground storage Tank #14 and Tank #15. - Tank #14 and Tank #15 were each 3K gallon compartments of one aboveground doublewall storage tank, registered in DEP FIRST as two tanks. Both tanks shared the same interstitial space. File photos from 1/14/15 before closure are attached in this report for reference. - Tank #14 (west end) had been used for diesel fuel, Tank #15 (east end) for UL gasoline. Tanks had been noted to be in good condition. - Piping was aboveground singlewall no contact with soil, tight fills were over one concrete fill spill containment, dispensing suction piping delivered fuel to separate free-standing dispensers. Dispensers and pans are intended to be used for installation of new tank, along with the piping above the pressure regulator/shear valves. No obvious integrity issues have been noted for the dispenser pans. - Per Glasgow Project Manager Jason De Gaglia, tanks were emptied and piping was blown back,_ manifests - No obvious petroleum odors, staining, or free product was observed on any of the concrete, soil, grassy or shrub areas at this time. - The TCAR Exemption Form was submitted by Leanne E.S. Cobb, FAU EHS Coordinator, 561-297-2385 (72385), 561-414-3224(cell), cobbl@fau.edu, and signed by Dennis Zabel, FAU EH & S Director. Form was filed, attached in FIRST.
