We are pleased to submit our proposal for the following: Con creto restoration repaks in accordance with ENGINEER'S and/or
INTERNATIONAL CONCRETE REPAIR INSTITUTE specifications and procedures.
This proposal provides an estimate based an visual inspections. Actual auantitles may increase or decrease and are based on
till following filed unit prices and estimuted agantiNes as follows:

Painting of repaired areas if required is not included in the estimated cost unless Itemized by a unit price (see exhibit "A" unit
price)
All material Is guaranteed to be as specified. Work will be performed In accordance with the drawings and specifications
submitted for above work and completed in a substantial workmanllke manner. Owner to provide electric and watorat no
cost to contractor, including any speclalvieetrleal set upo for swing stage "pig tells" and/or 220v service as necessary.
Contractor will remove debris and leave promises broom clean. However, dust Is an airborne nuisance and the contractor can
not be held liable for dust entry into units due to open windows by residence, broken or damaged windows, doors and other
areas due to wind farces and elements beyond the contractoes control.
A 5 YEAR WARRANTV IS PROVIDED FOR CONCRETE REPAIRS COMPLETED UNDER THIS CONTRACT. A
SEPARATE WARRANTY AS PER ATTACHED WILL BE PROVIDED BY any OTHER SUB -CONTRACTOR

Any disputes resulting from this agreement will be rattled by binding arbitration under the rules ofthe BETTER BUSINESS
AUREAU or other arbitration remedles as the parties may decide. If legal action is required in connection with the agreement
the prevailing party shell be entitled to recover its reasonable attorney fees.
