Kimley-Horn and Associates, Inc. has completed a post construction visual observation of the above
referenced project. The scope of work involved the replacement of antennas and the addition of
radios to an existing ground based installation.
This evaluation also makes use of digital photos of the as -built conditions taken after construction. I
hereby certify that the antenna and equipment installation at the site referenced above have been
performed in general conformance with the approved plans and specifications.
I hereby attest that the approved permit plans represent the as -built condition and envelope
components of the antenna and equipment installation.
To avoid any misunderstanding, I hereby state that, to the best of my knowledge, belief and
professional judgment, this report represents an accurate appraisal of T-Mobile's antenna and
equipment based upon careful evaluation of observed conditions to the extent reasonably possible.
