In addition to our on -site inspection, photographs of the installation of the antennas and mounts were taken to validate the proper installation of the equipment. Based on our personal review of the project site, to the best of knowledge and belief, the project has been constructed to the required specifications of the construction plans. Please see attached photographs of these construction elements of the project. Phone: 352281-2364 INSPECTIONS Type: Post construction field review Approved Building plans & Specifications referenced: St. Lucie County Permit #: 1512-0318 / 1512-0319 dated 1/25/16. Changes or modifications from plans: None ADDITIONAL INFORMATION CGN conducted site visit on Wednesday 3/16/16. CGN reviewed the site for compliance with the construction plans for the installation of the new antennas on existing antenna mounts. No deviations from the approved construction plans were noted. Coax lines were routed onto existing waveguide support structure and therefore, no additional structure was required. No additional exterior radio cabinets were installed on this project. Pictures of the completed condition of the site included. I hereby certify that the above referenced inspection was conducted completed on -site and the information presented in this report was,conveyed to the field personnel and the WAYPOINT Project Engineer.
