To Whom it May Concern,
Waypoint Engineering and Equipment LLC performed an on -site post construction site review
on June 14, 2016 to verify the antenna and coax cable installation at the above referenced
project location. In addition to our personal review of the site, Waypoint was provided additional
post construction photographic documentation of the completed construction by the general
contractor, J. Crompton Electric, Inc. Based on our review and contractor provided documents,
we find that the construction of the site is in substantial compliance with the permitted
construction documents.
Attached to this letter is our pictures of the post construction conditions at the project site.

Good morning Lydia,
To begin the email, Per our telephone conversation yesterday I would like to confirm that the color of
the Verizon Wireless cabinets will come in 2 options of colors, Tarnished Grey or Beige.
Also, in reference to your other comments.
1. "Please provide a copy of the lease agreement" - This is attached.
2. "Please provide a copy of the existing security bond" - I've attached the permits and notice of
commencements providing you information showing that there is no bond needed or required
between this property and Verizon Wireless.
3. In reference to the additional cabinet, the application and sub contractor form has been dropped
off at the permitting division, Permit number SLC- 1601-0134.
Will this release your hold for the permit to move forward to plan review? Please let me know if you
have any questions or concerns, My telephone number contacts are below. Once again thank you so
much for all of your help with this procedure.
