We are in receipt of your Request for Approval to Place a Wastewater Collection/Transmission System into Operation for the project authorized by the above referenced permit number the subject wastewater collection/transmission system extension has been constructed, and tested in accordance with the plans and related materials permitted by this agency, and is now ready for operation. Based on your engineer's certification of construction completion, equipment operation testing, if applicable, and the pressure & leakage test report for the above -referenced project, the project is hereby released for service in substantial accordance with the approved plans with the understanding that the project will be operated in compliance with Florida Administrative Code (F.A.C.) Chapter 62-604. Approval of individual connections to a completed collection/transmission system must be obtained from the City of Port St Lucie Utility Systems Department, separate of this notice by a Letter of Acceptance by the Utility Director or his designee. No. 132 This certification is based upon on -site observation of construction conducted by me or by a project representative under my direct supervision and upon review of shop drawings, test results/records, and record drawings performed by me or by a project representative under my direct supervision. The following is a description and explanation of substantial deviations* from the approved plans and specifications for the substantially completed portion of this project. 6" gravity service laterals off of the 8" gravity sewer main are approximately 23 linear feet shorter n the approved plans. This is due to the limited space in which cover over the 8" gravity main could graded back down to existing elevations at slopes not to exceed 4:1. The constructed cleanouts on se laterals (points of service) are now located on the 15' utility easement boundary. This deviation is expected to result in any adverse impacts to the operation of the gravity sewer system.
