Merejo, Director WASTEWATER CONSTRUCTION PERMIT Permittee: Nicholas Elliott Palm Park Center, LLC Address: 1497 SW Marin Downs Blvd. Palm City, FL 34990 RE: Coastal Floor and Granite Permit Number: WW2016-014 Date of Issuance: 07/07/2016 Date of Expiration: 07/07/2021 WWTF Connected To: Glades File Number: 5112 This wastewater construction permit includes 268 LF of 8" gravity sewer, 38LF of 6" PVC gravity line, and 2 manholes as shown on the construction plans stamped by PSLUSD on 6/13/2016 with page 5 dated 7/01/2016. The City of Port St Lucie Utility Systems Department has received authorization from the Florida Department of Environmental Protection (FDEP), under section 403.1815, Florida Statutes, F.S. per Final Order OGC Case Number 09-2886, to independently regulate the construction of sewage collection/transmission systems with mains of 12" or less. Therefore, a permit is not required by FDEP for this project. The referenced project was constructed prior to the above referenced FDEP permit expiring, however it was never certified. Therefore, the referenced project is hereby approved for construction of sewage collection/transmission systems facilities. Please be advised that this wastewater construction permit is subject to General Conditions of FDEP Rule 62- 4.160. F.A.C, as applicable and City of Port St Lucie Utility Systems Department (PSLUSD) (see page 2) and the following requirements: 1. If the sewage collection/transmission system being constructed under this construction permit was designed under the responsible charge of a professional engineer, then the permittee shall also retain a Florida -licensed professional engineer to take responsible charge of inspecting construction of the sewage collection/transmission system for the purpose of determining, in general, if the construction proceeds in compliance with this construction permit. 2. The permittee shall have complete as -built survey drawings produced for the sewer collection/transmission being constructed under this construction permit. The as -built drawings shall include the surveyor data of bell separation of all conflicts, as shown in the attached detail, along with pipe separation information and other pertinent data. An AutoCAD version of the as -built drawings shall be submitted with the certification package. 3. Upon completion of construction of the collection/transmission systems project, and before placing the facilities into operation for any purpose other than testing for leaks or testing equipment operation, the permittee shall obtain approval or clearance from the PSLUSD. Permits and/or approvals required by all state or local agencies must be obtained prior to the start of construction. B. The construction permit does not relieve the permittee from liability and penalties when the construction of the permitted activity causes harm or injury to human health or welfare; causes harm or injury to animal, plant or aquatic life; or causes harm or injury to property. It does not allow the permittee to cause pollution in contravention of Florida Statues and FDEP. C. The construction permit does not constitute a waiver from the requirements of the PSLUSD Utility Standards Manual and FDEP Regulations. All applicable sections from FDEP 62-604 shall be adhered to. D. The construction permit is valid only for the specific activity indicated. Deviations to the approved construction plans shall not be made without prior written approval of the PSLUSD. E. The permittee agrees in using the construction permit to make every reasonable effort to conduct the specific activity or construction authorized by the construction permit in a manner that will minimize any adverse effects on the adjacent property or on public use of the adjacent property, where applicable, and on the environment, including fish, wildlife, natural resources of the area, water quality or air quality. F. The permittee agrees in allowing required PSLUSD inspections as outlined in the PSLUSD Utility Standards Manual. G. The permittee agrees to maintain any permitted facility, or activity in good condition and in accordance with the plans submitted to PSLUSD as outlined in FDEP Rule 62-4.530 (1) F.A.C. and PSLUSD Utility Standards Manual.
