The contractor may delete the requirement of the one hour separation wall assembly .
between the new office area and the adjoining building, indicated on architectural sheets
A-1, A-2, A-3, and structural sheet S-2, `Building Permit Re -Issue", dated 06/03/16. The
contractor may install the shearwall "SW-l" framing indicated on section 1/A-3 in lieu of
the one hour separation wall assembly.

NOTICE OF RIGHTS
A party whose substantial interest is affected by this order may petition for an
administrative hearing pursuant to sections 120.569 and 120.57, Florida Statutes. Such
proceedings are governed by Rule 28-106, Florida Administrative Code. A petition for

entitled to judicial review pursuant to Section 120.68, Florida Statutes. Review proceedings
are governed by the Florida Rules of Appellate Procedure. Such proceedings may be
commenced by filing one copy of a Notice of Appeal with the Agency Clerk of the
Department of Health and a second copy, accompanied by the filing fees required by law,
with the Court of Appeal in the appropriate District Court. The notice must be filed within 30
days of rendition of the final order.

The site was visited by Jimmy earlier today. The applicant has two existing driveways, one is concrete, the other is
unimproved. In order to be consistent with the land development code, the non -conforming unimproved driveway
must be improved with either asphalt, concrete or brick pavers. Any culverts must be minimum 15" diameter and have
mitered end sections at the ends. A driveway permit will be required to be submitted to the water quality division. As
this is an existing driveway, the fee will be the $25 upgrade charge.
The owner has the option to abandon the non -conforming driveway, in which case, these improvements would not be
required.

See attached site plan for above property. They are renovation and partial enclosing the building. They were told that an
access/exit at Industrial Ave One is not allowed unless the road has been improved. Can you check if the existing
driveways are up to code? This is a commercial project for an auction house.
