Prima Vista Blvd.; Port Saint Lucie, FL Dear Sir or Madam: Pursuant to your request, Florida Engineering & Testing, Inc. (FE&T) has performed a field withdrawal resistance test in compliance with Testing Application Standard TAS-105 and the Florida Building Code High Velocity Hurricane Zone at the above referenced site. The purpose of our inspection was to determine the uplift capacity of the mechanical fasteners at the above referenced project. FE&T is a Miami Dade Certified Testing Laboratory (Certification #14- 1022.01). On April 13, 2016, our field representative visited the site and conducted ten (10) field withdrawal resistance tests on the 8" long Firestone Heavy Duty fasteners into the steel deck. All tests were found in compliance with the Florida Building Code and TAS-105. Enclosed find copies of TAS- 105 forms with test results. The test results presented reflect the condition of the roof system at the -time of the test. These results are time and sample dependent since roof conditions are continuously changing due to exposure to the elements. All tests were conducted according to Dade County TAS-105 using a calibrated Force -Test Fastener Tester Model No. SP2-2K (see attached calibration). Florida Engineering & Testing, Inc.
