Please be advised that Waypoint Engineering and Equipment LLC has reviewed the above referenced project site on
June 16th and has inspected the antenna and cable modifications to this 250-foot self-supporting tower. In addition
to our on -site inspection, photographs of the installation of the antennas and mounts were taken'to validate the
proper installation of the equipment.
Based on our personal review of the project site, to the best of knowledge and belief, the project has been
constructed to the required specifications of the construction plans.

ADDITIONAL INFORMATION
CGN conducted site visit on Thursday 6/16/16. CGN reviewed the site for compliance with the
construction plans for the Installation of the new antennas on existing antenna mounts. No deviations
from the approved construction plans were noted. Coax lines were routed onto existing waveguide
support structure and therefore, no additional structure was required. No additional exterior radio
cabinets were Installed on this project. Pictures of the completed condition of the site Included.
I hereby certify that the above referenced inspection was conducted completed on -site and the
information presented in this report was conveyed to the field personnel and the WAYPOINT Project
Engineer.
