'}:_�.PW This is to confirm my observation of roof slab construction on August 5, 2016. The purpose of the site visit was to observe the concrete after it was poured and tensioning of the post tension tendons. The concrete was poured more than 10 days before the site visit and it appeared to be in good condition with signs of cracks or deformities. The tendons were tensioned successfully. The purpose of the site visit was to observe the areas roof slab after the partial removal of the concrete by the contractor. The locations of the rebar and post tension tendons were exposed. The existing tendons were found in poor condition and some were completely severed. The contractor was instructed to remove and splice all tendons within the repair area and add restore the rebar reinforcement to original condition. Some additional rebar are to added at the edged. The contractor was instructed to proceed with repairs and follow-up site visit will be required prior to concrete pour. If you need additional information, please call me. Sincerely 7� Zuhair M. The purpose of the site visit was to observe the areas roof slab reconstruction after the rebar placement was completed and post tension tendons were spiced/repaired. The following inspections will be required: 1. Initial site visit at the time of cutting the concrete (demolition) 2. Observation of areas to be repaired after removal of deteriorated concrete. 3. Observe tendons tensioning 4. Final observation of area after repairs are completed and patched. All inspections shall be scheduled by the contractor with minimum of one work day notice.
