We are attempting to close the above referenced permit for concrete repairs to
the Empress condominium. The permit scope was initially made for repairs to a specific
number of units that were listed in the permit application. The units listed were from the
2nd floor up to the 15th floor. Actual repair work included the 1st and 2nd floors. We have
attached a matrix that shows the units included on the above permit and those that
need to added to the above permit.
Inspection reports were submitted with cover letters listing all the units where
repairs were made and that we inspected and approved. Attached are the reports and
lettWigned by the special inspector.

This letter is to inform you about the upcoming concrete restoration project at the above
mentioned address. All inspections, ongoing inspection reports, and final reports will be
completed by the special inspector of record. All inspection shall be performed in
accordance with FL Reg. 61G15-35. You should have received project specifications from the
contractor. We will be inspecting all the work during the project, including shoring, steel
reinforcement, forms, and concrete placements as follows:
Inspection Plan
• WORK LOCATIONS & DIRECTION: The engineer shall inspect structural repair areas
and direct work, as required, regarding the location and estimated extent of all structural
repairs.
• EXCAVATION LIMITS: The engineer shall inspect and approve, as required, all limits of
concrete removal and all steel reinforcement repairs.
• APPROVAL TO PLACE CONCRETE: The engineer shall inspect all areas prior to
concrete placement and give approval, as required, for all concrete placements. The
engineer shall inspect all prep work, including forms, shoring, safety, steel bar repairs,
and any adjustments to excavation limits.
• PLACEMENT OF CONCRETE: All placements of concrete require on -site engineering
and shall be inspected during the placements.
• FINAL: The engineer shall inspect and approve, as required, the completion of all
structural repairs for each phase of work.
