This letter is to inform you as threshold building special inspector, John Breitenbach PE 59770 will
be performing Threshold building services for the proposed Concrete Restoration Project at the
above referenced location, per the specifications and documents of CSM Engineers, dated May 27,
2016.
The inspection plan and scope of work are outlined in the CSM documents.
My threshold building special inspector number is 2072; firm registration #26001.
I will be sending you copies of the final inspection. reportlor final completion certification letter when
the project is completed (as this is a small restoration project). The following general sequence of
inspection work:
1. Inspection of concrete surface after demolition or spall removal.
2. Inspection of forming, shoring, bar coating, re -bar replacement, concrete slump, etc.....
3. Concrete pouring and final completion of repair areas.
