Pierce, Florida CONTRACTOR: Structure -Con PROJECT MGR: Khris McConnell INSPECTED BY: Mike Luc WEATHER: Overcast slight rain WORK INSPECTED: Building A — North Building Balcony repairs — Unit 508 Demolition of the concrete was observed at several locations which included the following: 1. Surface repair along the exterior at the south sliding glass door. This appeared to be loose decorative topping on a previously repaired area and no structural damage to the slab. 2. Slab edge of the south facing balcony. There was some corrosion of the reinforcing and no post tension cables are affected, repair as per the documents 3. Full depth repair in the middle of the slab on the south side. This was caused by a corroded post tension splice coupler that was not encapsulated or taped. The coupler should be mechanically cleaned, coated with a corrosion inhibitor and wrapped with a PVC tape. Do not excavate any further behind the bearing end of the anchor. 4. Column repair at the north end. There is corrosion of the reinforcing at the bottom of the column which extends down into the columns. Further excavation into the slab is required to repair the column as per the documents. Re-inspedt prior to concrete placement. M L Engineering, Inc. Pierce, Florida PROJECT MGR: Khris McConnell WEATHER: Overcast slight rain Unit 208 1. There are 3 post tension cables that Be completely corroded and broken in the upper surface of the balcony slab. The pt anchors at the balcony edge are set back in about 12 inches from the slab edge with no pockets. The cables appear to have been tensioned from the other end of the building on the west side. 2. The anchors on the west elevation are to be located and the cables replaced. The slab should be shored when excavating the balcony slab to place the new section of cable. Unit 508 1. The reinforcing and post tension splice coupler have been coated and taped and are approved for concrete placement at the repair areas. 2. Anchors for the shutter track and guard rail clip should be 1 maximum in length to avoid the possibility of damaging pt cables or touching the column reinforcing. Unit 808 1. There is a large ceiling spall at the underside of the 908 slab above that requires repair that was not on our original list. The two 2 sq ft surface spalls on the list do not require repairs. The edge crack on the southern edge needs as well as the column at the north side needs excavation and repair. Unit 1001 1. The spall at the door threshold has been excavated and found to be loose topping. The concrete needs to be chipped down further to allow a minimum repair mortar thickness of about 3/8" for adequate bonding. 2. Concrete has been chipped out at the edge spalls on the north and east balcony edges. There are 2 pt anchors on the east and 1 pt anchor on the north balcony edge. There is some corrosion of the pt anchors and bursting bars. The existing reinforcing bars perpendicular to the slab edges were too long and are corroded. 3. Continue cleaning the concrete from the top of the bursting bars just enough to verify if the extent of corrosion and without affecting the live anchors. M L Engineering, Inc. If there is less than 20% loss of section, the bars and anchors can remain and repaired, if not, the cables are to be locked off, de -stressed and replaced per the specs. Concrete has been chipped out at the edge spalls on the north and east balcony edges. The reinforcing and pt anchors have been cut back, cleaned and coated with corrosion inhibitor on the east edge. Remove the excess paint from the anchor insert to allow the greased end caps to be placed. 3. The anchor and reinforcing is slightly high in the slab but they cannot be moved deeper in to the slab unless other unaffected bars are removed. Clean the reinforcing bars and anchors and repair the cable end and concrete. 4. There is an area of approximately 9 sq feet of ceiling and edge spall at the northeast corner. This may require locking off at least one cable at the north edge. Contractor to verify if there are other cables along the adjacent east prior to chipping out the concrete. Unit 1006 5. The corrosion has been chased and all bars have been cleaned, primed and ready for pour. The total area of ceiling repair is 19 square feet. 2. The edge crack at the southern edge needs excavation and repair. Unit 1001 1. The northern slab edge has been excavated and one pt cable has been locked off. The existing reinforcing bars have been cleaned, primed and ready for pour. The total area is 5 linear feet. 2. The northeast slab edge comer is in progress. One pt cable was discovered and will require locking off prior to further excavating. Unit 1101 1. The northeast corner has been excavated and one pt cable locked off. The existing reinforcing bars have been cleaned and primed. The end of one bar running in the north to south direction will require cutting to provide an edge clearance of 1 '/2". The new anchors were placed all the way out to the edge so they can be stressed. Provide 2 #4 bursting bars behind the anchors at the slab edge. Bars need to be epoxy anchored in the middle of the slab. Since the slab is only about 4 to 5 inches thick the bars should be offset and should not be aligned vertically. 2. There were existing splices in the same 3 cables which had to be excavated to so that the new cables can be tensioned. 3. Tape all exposed post tension cables and connections. Unit 1001 1. Concrete repair needed at NE corner of balcony. There is one pt cable that had to be locked off and will require to be spliced and re -tensioned. 2. The repair to the cable at the north edge has been completed. Cable spliced, re -tensioned and encapsulated, ready for concrete repair mortar. Unit 1101/1001 1. The 2 cables at NE corner of the 1101 floor slab (1001 ceiling) have been repaired, re - tensioned and first concrete pour completed. The second pour is now ready. All anchors for the re- installation of the door track shall be stainless steel anchors in holes sealed with a polyurethane sealant with a maximum embedment of 1" so as to avoid damaging pt cables Unit 1001 1. The one pt cable has been locked off and spliced. 2. Provide a 90 degree corner lap bar with 12 inch legs each way to tie the edge bars. Two additional pt cables along the north slab edge required repair due continued corrosion of the reinforcing bars behind the cables along the slab. 3. The reinforcing bars have been cleaned and coated with rust inhibitive primer. The cables have been locked off and spliced. The first pour at these to cables is approved in order to re -tension the two cables and will allow the lock to be removed and then the final pour once the concrete has achieved a minimum of 3500 psi 4. The spall at the door threshold has been repaired. 2. Concrete has been chipped out at the edge spalls on the north and east balcony edges. The reinforcing and pt anchors have been cut back, cleaned and coated with corrosion inhibitor on the east edge. Remove the excess paint from the anchor insert to allow the greased.end caps to be placed. 3. The anchor and reinforcing is slightly high in the slab but they cannot be moved deeper in to the slab unless other unaffected bars are removed. Clean the reinforcing bars and anchors and repair the cable end and concrete. 4. There is an area of approximately 9 sq feet of ceiling and edge spall at the northeast corner. This may require locking off at least one cable at the north edge. Contractor to verify if there are other cables along the adjacent east prior to chipping out the concrete. Unit 1006 5. Surface repair along the exterior at the south sliding glass door. This appeared to be loose decorative topping on a previously repaired area and no structural damage to the slab. 2: Slab edge 6f the."outh facing balcony. There was some corrosion of the reinforcing and no post tension ;cables are affected, repair as per the documents 3:' Full depth repair -in the middle of the slab on the south side. This was caused by a corroded post tension splice coupler that was not encapsulated or taped. The coupler should be mechanically cleaned, coated with a corrosion inhibitor and wrapped with a PVC tape. Do not excavate any further behind the bearing end of the anchor. 4. Column repairat the north end. There is corrosion of the reinforcing at the bottom of the column which extends down into the columns.
