Building Official:
This letter is to inform you as threshold building special inspector, John Breitenbach PE 59770 will
be performing threshold building services for the proposed concrete balcony repair. Breitenbach
Engineering will also act as the Engineer of Record.
Structural work will involve 21 If +- of slab edge repair. Other miscellaneous work may include
stucco repair, painting and sealing of the repair work.
Specifications: (see attached)
1. All work performed per the International Concrete Repair Institute (ICRI), Guideline #03730
2. Use Sika 211 grout repair.
I will be sending you copies of the final inspection report/or final completion certification letter when
the project is completed. The inspection plan is as follows:
1. Inspection of rough demolition..
2. Inspection of rebar, coatings, forming, and shoring prior to pour.
3. Final painting & finish completion.
Breitenbach Engineering will be working directly for the condo association as FOR and specialty
inspector. Drawings not made for minor repairs. See plan sketch attached:

This letter is to inform you as threshold building special inspector, that the Contractor has
satisfactorily completed the concrete balcony repair per the specifications of. Breitenbach
Engineering, dated October 12, 2016_ .
The attached sketch shows the final full depth repairs. Demolition and final repairs are different than
the preliminary sketch previously submitted prior to destructive investigation. Please see the revised
areas and locations.
Photos were taken by the Contractor, and final inspection was made on 12/1/16, passed.
