Ault Morrison Hershfield has completed a post construction visual observation of the above referenced project. The scope of work under permit # SLC-1611-0211 involved the replacement and addition of remote radios to an existing guyed tower. This evaluation makes use of digital photos of the as -built conditions taken after construction. I hereby certify that the remote radio installations at the site referenced above have been performed in general conformance with the approved plans and specifications. The approved permit plans represent the as - built condition and envelope components of the antennas and ground equipment. To avoid any misunderstanding, I hereby state that, to the best of my knowledge, belief and professional judgment, this report represents an accurate appraisal of T-Mobile's equipment based upon careful evaluation of observed conditions to the extent reasonably possible.
