SCOPE:
This NOA is being issued Under tile applicable rules and regulations governing tile use of construction niatcrials.The
dOCUnientation submitted has been reviewed and accepted by Mianii-Dade County RER - Product Control Section to be
Used in Miarni Dade County and other areas where allowed by the Authority Having Jurisdiction (AHJ).
This NOA shall not be valid after the expiration date stated below. Tile Mikkiiii-Dade County Product Control
Section (In Miami Dade County) and/or (lie AM (in areas other than Mianii Dade County) reserve the right to
have this product or material tested for quality assurance put -poses. If this product or material fails to perform in
tile accepted manner, the nianufactuier will incur the expense or such testing and tile AM may itrallediitely
revoke, modify, or suspend the use ol'such product or material within their.jurisdiction. RER reserves tile right to
revoke this acceptance, if it is determined by Mianni-Dade County Product Control Section that (his product or
material Fails to meet the requirements of the applicable building code.
This product is approved as described heleill, and has been designed to comply with the Florida Building Code
including the High Velocity Hurricane Zone of the Florida Building Code.

For sales, advertising or any other purposes shall automatically terminate this NOA. Failure to comply with any section
of this NOA shall be cause for termination and removal orNOA.
ADVERTISEMENT: Tile NOA number preceded by tile words Mianni-Dade County, Florida, and followed by tile
expiration date may be displayed in advertising literature. If any portion ofthe NOA is displayed, then it shall be (lone
in its entirety.
INSPECTION: A copy of this entire NOA shall be provided to tile user by the manufacturer or its distributors and
shall be available for inspection at thejob site at tile request of tile Building Official.

1.2 Any other documents required by the Building Official or the applicable code in order to properly
evaluate the installation of this systern.
