This letter shall serve as verification, in accordance with our agreement as to the extent of our
services in this regard, that the Verizon upgrade is substantially complete.
The site is located at 7370 — 7380 South Ocean Drive, Jensen Beach, FL 34957. (6) Existing antennas
have been removed and replaced with (6) new antennas. (6) Diplexers have been installed on the
handrail behind the existing battery cabinet. (6) Triplexers have been installed on the handrail behind
the existing battery cabinet. (12) Diplexers have been installed on the antenna pipe mount pipe. (6)
bias-t's have been installed on the antenna pipe mount. There was no electrical scope of work
associated with this permit.
The work has been reviewed and the work is substantially completed in accordance with 5`s edition of
2014 FBC and the permitted plans.
