Based on photographs and signed as -built construction documents provided to CBVR
Telecom Design Group by the contractor (Anthony Ankersmit — UCI Construction
Services, LLC) on December 6, 2017, I hereby attest to the best of my knowledge, belief,
and professional judgment that the installation of wireless communications equipment
for the aforementioned project was constructed in general conformance with the
approved civil construction documents by CBVR Telecom Design Group, dated March
21, 2016:
• Removal of (6) antennas
• Installation of (6) new antennas
• Installation of (6) 7/8" coax and (4) 1-5/8"
• Installation of mount reinforcements
• Installation of (3) bias-t's at antenna mounts
The applicable codes to this installation include:
• Florida Building Code 2015 (5`h edition)
• ASCE 7-10 (ult. wind speed = 159 mph, risk cat. = II, exp. = D)
• ANSI/TIA-222-G (nom. wind speed = 124 mph, struct. class = II, topo cat. = 1)
• Florida Fire Prevention Code (2014 edition)
• National Electric Code (NFPA 70-2014)
• Local City or County Ordinances

the information, which is based on the documentation described above, shall not be
guaranteed. Furthermore, this statement shall not constitute approval of safety
precautions or of any construction procedures.
