By: Date:.r�1`% Date: CONDITIONS OF DRIVEWAY CONSTRUCTION If granted a permit for construction of the proposed driveway on Countyright-of-way, we agree to abide by the following: 1. Setbacks Due to design constraints, any driveway providing access to a one or two family dwelling, shall normally be a minimum of six (6) feet from the nearest side property line without culvert and nine (9) feet with a culvert. In the case of comer lots, driveways shall be constructed so that the center line of the driveway is a niinimum.of twenty (20) feet from the parallel base building line (right -of -way -line). 2. Safe -All construction work shall comply with the safety provisions of all Federal, States, and local laws. 3. Traffic Control -Measures shall be provided during construction in accordance with the 'Florida D.P.T. manual on Traffic Control and Safe Practices. 4. To Save Harmless -The Board;of County Commissioners and each and every ,thereof from the payment of any compensation or damages resulting from the exercise of rights and privileges herein granted. 5. Culvert Installation -Culvert installation shall be accomplished in accordance with the provisions of this permit, and shall be the responsibility of the applicant. 6. Removal of the Installation -In the case of an applicant's noncompliance with:the requirement's stated herein, the installation will be promptly brought into.compliance. Inspection of the work does not relieve the applicant from compliance with the permit conditions. .7. Riaht-of-Way Limitations -Sprinkler systems, sidewalks, driveway or street side boarders, drains from private property, electric lighting, signs, landscaping, other than breakaway type Mailbox posts, parking facilities and crosswalks or anything that would hinder the county' maintenance operations, shall not be placed and/or constructed in the County right-of-way. 8.. Concrete or Asphalt Paving.Paving a driveway within the.County right-of-way is allowed only when the roadway to which it will join is paved. 9. Repairs of Improvements by461icant-The applicant agrees to promptly repair any damage or injury to improvements on Ceunty right-of-way caused byreason of the exercise of rights and privileges herein granted, restoring same to a condition equal to that which existed Immediately prior to the, infliction of such damage or injury in a manner satisfactory'to the County. 10. Time Limitations -Al work must be completed within six (6) months from the date of issuance or this permit will become null and void. 11. In the Right -of -Way Area•Ddveways shall be designed to be perpendicular to.the — - roadway to which they join and be centered on the culvert. 12. Inspections -A pre -pour inspection shall be completed:and approved by a County -inspector prior to concrete or asphalt being placed in the County right-of-way. A final inspection shall be,completed and approved by a County Inspector when all the. conditions of the permit are met. A minimum ,twenty-four(24) hour notice -is normally required prior to an inspection being scheduled. I- 13.. Asphaltic concrete driveway extending into right-of-way or public easement shall consist of 1" Type I I Asphaltic Concrete centered on a 12" wider base of 4" thick compacted lime rock or coquina base, density shall be 98% of maximum as determined by AASHTO T-180. 2.- Right -of -Way Restoration: a. Driveway Permits td'be.applied for through the Planning and Development Services Department, although - Driveway Permits are issued through the Water Quality Division. 2. All. Payments for Driveway -Permits; including reinspect fees., should be made in the Planning and Development'Services Department. The Water Quality Division cannot receive payments. 3. If •'a .
